The Service Coordinator I provides mental health service coordination, according to the level of need, to a population of individuals with serious mental health diagnosis and substance use disorder. The primary function of Service Coordination is to provide professional assessment, service planning, service coordination, referral, and re-evaluation services required for a safe and healthy community life which is manifested through stability in relationships, housing, and employment. This is done in accordance with the Agency's vision, mission, and values, and emerging mental health Recovery Principles. This position provides an essential recovery orientation for those suffering with SUD.

*This position is grant funded and is not eligible for on call rotation*

This position is eligible for a sign-on bonus of $3,000 for an 18-month work commitment!

Responsibilities:

Care Coordination & Service Planning

  • Coordinate substance use services, support recovery goals, and develop person-centered service plans.
  • Assess individual and family needs, facilitate service planning meetings, and ensure services are delivered effectively.

Resource Linkage & Advocacy

  • Connect individuals and families to community resources and supports.
  • Advocate for individuals by removing barriers, coordinating services, and promoting access to needed care.
  • Provide culturally responsive services that respect diverse backgrounds and identities.

Documentation & Compliance

  • Maintain accurate records, complete required documentation, and ensure regulatory compliance.
  • Support continuity of care during hospitalizations and transitions.
  • Participate in ongoing training to stay current on best practices and recovery-oriented care.

 



  • Bachelor’s Degree in a Human Services field such as Social Work, Psychology or Counseling required.

  • Other fields of study may be considered with coursework of at least 12 credits in Human Services.

  • One-year related experience preferred.

  • Prior experience working in community settings preferred.

  • Safe driving record required.

Licensure, Certifications, and Clearances: 

  • Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
  • Cardiopulmonary Resuscitation (CPR)
  • Comprehensive Crisis Management (CCMC)
  • Driver's License
  • Act 31 Child Abuse Reporting with renewal
  • Act 33 with renewal
  • Act 34 with renewal
  • Act 73 FBI Clearance with renewal
