Position: VRP SSG Fox Case Manager

Summary of Job:

The VRP Case Manager is responsible for conducting clinical assessments, care coordination, and community engagement services. The Case Manager provides crisis intervention, develops individualized service plans, and coordinates referrals while maintaining the highest ethical standards and representing Easterseals in the community.

Skills and Requirements:

  • Bachelor's Degree in Social Work or related field required
  • Military service preferred
  • Experience in Crisis Intervention and Suicide Prevention programming
  • Strong background in case management and care coordination
  • Experience with community outreach and relationship building
  • Excellent project management and organizational abilities
  • Strong conflict management and trauma-informed de-escalation skills
  • Proficiency with Microsoft Office Suite
  • Self-motivated with ability to work independently and collaboratively
  • Strong attention to detail while managing multiple priorities
  • Valid driver's license with proof of insurance; willingness to use personal vehicle
  • Flexibility to work early mornings, evenings, and weekends as needed
  • Commitment to maintaining confidentiality and professionalism
  • Passion for Easterseals' mission and ability to serve as an organizational ambassador
  • Familiarity with crisis intervention techniques and crisis management strategies
  • Proven ability to work collaboratively with multidisciplinary teams and diverse populations

Position Duties and Responsibilities 

  • Conduct suicide and risk assessments
  • Facilitate initial care assessments to determine treatment priorities
  • Create and manage Individualized Service Plans (ISPs)
  • Deliver Skills for Psychological Recovery (SPR) curriculum
  • Focus on skill-building components including problem-solving, positive activities, managing reactions, helpful thinking, and social connections
  • Coordinate referrals and facilitate warm handoffs to VA facilities
  • Ensure seamless transition and continuity of care
  • Build relationships with target area service providers
  • Develop and maintain community partnerships
  • Travel 25-50% of time throughout service area
  • Attend local veteran events across target area
  • Represent Easterseals and Veterans Rally Point in the community
  • Participate in outreach activities
  • Maintain highest ethical standards in all interactions
  • Support special projects as needed
  • Conduct thorough assessments of clients' needs and develop personalized care plans
  • Coordinate services across various sectors including healthcare, social services, and community resources
  • Provide crisis intervention and management to clients in urgent situations
  • Monitor client progress and adjust care plans as necessary to ensure effective support
  • Facilitate communication between clients and service providers to advocate for their needs
  • Maintain accurate records of client interactions, progress notes, and service referrals
  • Supervise support staff or interns when necessary to ensure quality service delivery
  • Stay informed about relevant policies, regulations, and best practices in social work and case management
  • Perform and/or supervise other job-related activities as may be assigned.
  • Community Outreach – community presentation on suicide- community education suicide awareness

Physical Demands Efforts 

This function requires moderate standing and walking with extended periods of sitting. Will be sitting while performing computer functions. Position does not involve climbing or crawling but may on occasion kneel/squat. Ability to operate normal office equipment such as, telephone, copy machine, fax, scanner, and computer. Position requires ability to frequently push/pull/lift objects more than 50 pounds, see objects closely and to hear normal sounds with some background noises. 

Mental & Visual Demands

Specific vision abilities required by this job include close vision, distance vision and ability to adjust focus. Position requires ability to concentrate on fine detail with interruption and attend to tasks for more than 60 minutes at a time.

Work Environment and Hazards

This position typically works in an office environment. Noise is moderate and typical of conversations and office equipment.
