What You'll Do

  • Advocate for residents' social, emotional, and psychological well-being
  • Support residents and families throughout their stay and discharge planning process
  • Coordinate care and resources with residents, families, and community partners
  • Participate in interdisciplinary care planning and resident assessments
  • Help ensure residents receive the support and services they need to thrive
  • Promote resident rights, dignity, and quality of life

What You'll Need

  • Bachelor's degree in Social Work 
  • Two years of social work experience required
  • Skilled nursing or healthcare experience required
  • Knowledge of state and federal regulations
  • Strong communication, assessment, and documentation skills
  • Ability to work collaboratively with residents, families, and interdisciplinary teams
