As a Registered Nurse Case Manager with Guthrie Home Health, you'll have the opportunity to care for patients in the comfort of their own homes while enjoying greater independence, flexibility, and autonomy than many traditional nursing settings offer.

In this role, you will serve as a trusted clinical resource for patients throughout their home health journey, providing skilled nursing care, coordinating services, educating patients and families, and helping individuals safely manage their health needs outside of the hospital. Every day offers the chance to make a genuine impact while developing lasting relationships with the patients you serve.

This position is based out of our Waverly Home Health office and includes travel throughout the service area. Nurses enjoy a primarily weekday schedule with the ability to independently organize patient visits while remaining connected to a collaborative team of clinicians, therapists, social workers, and support staff.
 

Schedule Highlights

  • Full-time, Monday through Friday
  • Typical hours: 8:00 AM to 4:30 PM
  • Shared on-call responsibilities
  • Weekend rotation approximately once per month
  • Flexible daily scheduling based on patient needs and caseload

 

Home health experience is preferred but not required. Nurses with strong assessment skills, sound clinical judgment, excellent communication abilities, and a passion for patient-centered care are encouraged to apply.

Position Summary: 

Responsible for the case management and provision of quality home nursing services to patients within the agency’s philosophy, policy, and Standards of Community Health Nursing Practice.

Education, License & Cert: 

Graduation from an approved School of Nursing, Bachelor’s Degree in Nursing preferred.

Experience: 

One‐year previous nursing experience with Medical/Surgical Nursing required.  Prior Public Health/Community Health experience preferred.

Essential Functions: 

  1. Plans, implements, manages, and evaluates the provision of both professional and ancillary home health services (direct care and case management) to ensure that all patient’s needs are met and quality care provided in accordance with Federal, State,  and program guidelines. 
  2. Is responsible for ensuring that he or she can meet the yearly competency requirements. Attends staff meetings and educational opportunities. 
  3. Must provide nursing care in the patient’s environment. 
  4. Manages the activities of the multi‐disciplinary team providing care to patient. 
  5. Demonstrates effective time management, flexibility and priority setting in patient care. 
  6. Must have excellent verbal and nonverbal communication skills and facilitate communication between team members and patients. 
  7. Independent judgement and excellent planning and problem‐solving skills are required. 
  8. Is responsible for LPN supervision. 
  9. Must have reliable transportation. 
  10. Must be able to accept on call assignments.

The payrate for this position is $40.00 - $54.12 per hour.
