Title:  Social Services Coordinator

FLSA Status: Non-Exempt, Full-Time / 37.5 hours per week

Work Schedule: Monday - Friday, 9AM - 5PM

Wage Rate: $30 per hour, depending on experience

Sedgebrook is recruiting for a hospitality focused Social Services Coordinator to join our team! This position is responsible providing social work services to the residents and family members of the Health Care Center.

Essential Job Duties:

  • Assists in completing psychosocial assessments, MDS and other required documentation in accordance with department policy, and State and Federal regulations. 
  • Assists in providing psychosocial support to residents and families with emphasis on new residents to ensure successful adjustment to the Health Care Center.
  • Participates in the development of the individual's care plan including the goals and approaches for the Social Services component
  • Coordinates discharges/transfers to ensure the appropriateness and continuity of care. Develops a comprehensive discharge plan in coordination with residents/families and multidisciplinary staff.
  • Provide/identify referral, agency and/or community resources to meet the needs of the resident/family.
  • Maintains regular and on-going relationship with family to discuss needs or concerns, mediates issues that may arise between resident/family/staff.

Qualifications:

  • Bachelor's degree in social work or human services field is preferred. 
  • Experience in a skilled nursing facility (SNF) required.
  • One year of social work experience preferred in long term care/assisted living. Knowledge of geriatric population and dementia. 
  • Excellent oral, written and interpersonal skills. Must present a professional manner and be able to deal with situations that may be stressful.
