What You’ll Do
♦ Provide social services support to residents and families in compliance with state, federal, and facility standards
♦ Participate in admissions, care planning, and discharge planning.
♦ Complete and maintain resident social histories and progress notes.
♦ Serve as a liaison between residents, families, staff, hospitals, and outside agencies.
♦ Provide emotional support to residents and families, including during crises.
♦ Assist with medical appointments, transportation, equipment needs, and personal requests.
♦ Monitor changes in resident condition, report incidents, and assist during emergencies.
♦ Follow HIPAA guidelines and facility policies.
♦ Perform other duties as assigned.

What You’ll Need
♦ Bachelor’s degree in Social Work or a related Human Services field.
♦ Ability to assist residents physically, including pushing wheelchairs and repositioning.
