We are hiring a Clinical Care Specialist, to join our Family Center team of professionals at 1591 Taos Rd in Pueblo.

                                               

What You’ll Be Doing

 

As a Clinical Care Specialist with The Family Center, you would play a central role in helping our clients’ access services and resources, and take the next steps to wellness. You would use your professional case management skills to assist clients in managing and overcoming their symptoms and difficulties to improve day-to-day functioning, support other members of the treatment team, and collaborate with professionals within and outside our agency to assure that clients receive the very best care available.

 

RESPONSIBILITIES: As a Clinical Care Specialist under the supervision of the Director and Supervisor you will be:

 


Utilizing High Fidelity Wraparound principles and strategies to help support children, youth and families
Providing Behavioral Health Counseling to children, youth, and their families
Meeting clients in the office, community, and/or their homes
Identifying stressors and precursors to crises
Identifying coping mechanisms and natural supports that may be available
Provide education regarding co-occurring diagnoses
Enhancement and generalization of problem-solving skills
Providing a consistent support and availability until the consumer has achieved a higher level of functioning
Linking the client with community resources
Helping the client to make and keep scheduled appointments with medical doctors
Providing/arranging transportation for scheduled appointments
Providing support to reduce usage of/divert from more restrictive treatment settings or jail
Keeping accurate, complete and up-to-date records with 100% compliance with company standards
Maintaining competency and proficiency with the agency’s Electronic Health Record
Performs other job-related duties assigned by the director or his/her designee


 

Physical requirements include the following:


Frequently stays stationary for prolonged periods of time
Frequently moves within and between facilities
Frequently exposed to stressful or emergent situations
Frequently operates a computer and other office equipment to record health data
Constantly communicates with clients and others
Occasionally moves equipment and/or materials up to 20 pounds
Occasionally drives between Health Solutions facilities for other assigned duties

QualificationsWhat we’re looking For—The Must-Haves

 


Bachelor’s degree in social work, psychology, criminal justice or a closely related field
Experience in case management/community systems navigation, particularly with the criminal justice and/or mental health systems.
Experience working with individuals with substance use/co-occurring disorders services
Experience working with persons diagnosed with severe & persistent mental illness
Exceptional flexibility, time management and documentation skills
Must be a self-starter and self-motivator
Computer proficiency
Colorado driver’s license, a good driving record, and access to transportation during work hours
