Primary Function 

Position will serve as the Supervisor for the Medication Assisted Treatment (MAT) Program for CIHA Behavioral Health. The Medication Assisted Treatment Program is the use of medications with counseling and behavioral therapies to treat substance use disorders and prevent opioid overdose. Supervisor will provide clinical support, administrative, and clinical supervision for Medication Assisted Treatment Program staff. The clinical supervisor will oversee the documentation and professional issues of Opioid Treatment Program staff. This position will review and assess client treatment plans, including short- and long-term goals and clinical records, to evaluate the necessary level of patient care. This position will primarily serve adults but may also see families and children. This position will be 70% clinical and 30% administrative (subject to change depending on program need). Services include strength-based comprehensive clinical assessments of adults and children, implementation of appropriate evidence-based treatments for psychiatric and substance use disorders when indicated, case management services, and collaboration with other professionals in an integrated multidisciplinary environment. Employee is expected to provide individual, family, and group therapy utilizing culturally appropriate and evidence-based modalities of treatment. The employee will provide emergency, crisis intervention, consultation, and education services as needed.

Supervisory Duties

  • Provide clinical and administrative supervision to Medication Assisted Treatment Program staff.
  • Position is expected to be 70% clinical and 30% administrative (subject to change depending on program need)
  • Take direction from the Medication Assisted Treatment Manager and the Director of Adult Outpatient Services.
  • Understands the harm reduction philosophy of treatment and portrays this in leadership.
  • Ensure CIHA meets all applicable standards and requirements.
  • Ensure CIHA Opioid Treatment Program meets or exceeds productivity and revenue targets.
  • Ensure knowledge of all applicable CIHA/Analenisgi Policy and Procedures.
  • Provide clinical oversight for all patients served with BH and substance use needs who are enrolled in the Medication Assisted Treatment Program.
  • Provide clinical supervision to staff as needed.
  • Facilitate weekly multidisciplinary and cross-agency staff meetings, which may involve CIHA behavioral health and interdepartmental staff and stakeholders.
  • Ensure the Medication Assisted Treatment Program is taking measures to improve practice by completing performance improvement projects in areas needing improvement.
  • Inform leadership of the needs of the Opioid Treatment Program as they arise and make recommendations to resolve
  • Participate in leadership/supervisory training and activities to enhance competencies in areas as needed or assigned.
  • Participate in completing annual performance appraisals of direct reports with the Adult Service Manager.
  • Ensure quality evidence-based treatment interventions are utilized with clients.
  • Collaborate with all BH teams, including, but not limited to, the BH child/adult services, psychiatry, nursing, behavioral health clinical director/assistant director, peer support, and targeted case management.
  • Ensure collateral (family/supports) involvement in treatment/safety planning for the patient when appropriate.
  • Ensure timely, accurate, and complete communication to residential treatment staff and interdepartmental teams on transfer of patients to Kanvwotiyi.
  • Ensure clinical data is collected and reported as required according to the (BH, ER, and Inpatient) department’s specific needs and CIHA policy.
  • Monitors and maintains utilization management of any services, as determined.
  • Ensure documentation of all treatment and clinically significant events in the Electronic Health Record as prescribed by CIHA policies and procedures and quality assurance standards.
  • Communicate regularly with the Behavioral Health Director, Behavioral Health Assistant Director, Adult Services Manager, Child Services Manager, CIHA Executive staff, and other hospital departments as needed.
  • Provide staff training as directed, according to expertise and scope of practice.
  • Participates in the provision of periodic in-service training for non-health care practitioner members of the clinical and administrative staff on subjects germane to clinical and case management services. This shall include responsibility for developing and implementing psycho educational programs, community relations, and information and referral.
  • Participate in completing performance appraisals of direct reports with the applicable manager/supervisor.
  • Participates in division/program meetings to review and identify opportunities to improve the program and treatment provided, and recommends corrective action when problems are identified. This includes assisting with changes in policy and procedure, which would improve the care of service delivery
  • Assists in attaining and maintaining accreditation by the Joint Commission of Health Care Organizations.
  • Ensures implementation of provisions contained in all applicable privacy/confidentiality policies, procedures, and practices for maintenance and release of patient records and clinical information, including EBCI, CIHA, IHS, NC DHHS, HIPAA, and The Joint Commission
  • Assist with the recruitment and hiring process for employees.
  • Develop and maintain collaborative relationships with community stakeholders.
  • Assist in other areas of the BH department when needed.
  • This position requires direct service duties with time allocations given to complete supervisory duties.
  • Participate in a rotation for on-call crisis services as part of the BH Team
  • Other clinical and administrative duties as assigned.

Direct Service Duties

  • Provides services to EBCI and other eligible persons on a primarily outpatient basis with some inpatient consultation. Responsible for completion of intake forms, identification of community resources, patient comprehensive clinical assessments, treatment plans, provision and coordination of supportive services, and patient advocacy.
  • Provides therapy services to patients in exceptionally difficult cases where such is appropriate, as determined by treatment needs.
  • Assess and treat clients in individual, group, couples, and family modalities, exercising professional, clinical judgment and using a wide range of therapeutic skills to assist patients and their families in dealing with chronic and acute MH/SUD diagnoses and trauma.
  • Cooperates and collaborates with the medical staff and other health professionals/agencies in developing a viable therapeutic team network to facilitate and establish patterns of cooperative efforts and to facilitate patient care, discharge, and follow-up.
  • Provide counseling to patients and family in matters directly related to patients' limitations, finances, adjustments to medical conditions, and ongoing treatment.
  • Implements and follows the provisions contained in all applicable privacy/confidentiality policies, procedures, and practices for maintenance and release of patient records and clinical information, including EBCI, CIH, IHS, NC, DHHS, HIPAA, Joint Commission, CFR 42, or other licensing bodies.
  • Responsible for participating in clinical staffing as an integral component of the Opioid Treatment Program team, facilitating the discussion of patient data, and formulating changes to the treatment plan as clinically indicated.
  • Responsible for the development of individual treatment plans, including the development of time-limited, quantified, measurable, and salient goals, and prescribing procedures and interventions that facilitate goal attainment. Treatment plans must be consistent with the patient's diagnosis, agency quality assurance guidelines, and level of care policies.
  • Shall perform limited administrative duties, which include maintaining statistical records of clinical workload, utilization review, and quality assurance, participating in education programs, and participating in clinical quality assurance functions.
  • Assists in attaining and/or maintaining compliance with CIHA, Joint Commission, IHS, Medicaid, SAMHSA, other required governing bodies, and funding sources
  • Timely record all treatment and clinically significant events for the medical record in the manner prescribed by CIH policies and procedures, quality assurance, quality improvement, and documentation manuals.
  • Provides written, telephone, and face-to-face consultation services to various persons and organizations that affect the patient. These include schools, courts, probation officers, physicians, hospitals, medical care providers, law enforcement officers, social services and community agencies.
  • Represents the Medication Assisted Treatment program at regular team meetings for progress review and during emergency meetings dealing with high-risk situations
  • Utilize training and skills in community organizations, participate in community planning, and implement programs to help address such complex issues as improving coordination of services or developing programs for individuals with intellectual and developmental disabilities, and persons with substance use disorders.
  • Expected to obtain continuing education both through in-service training and through access and tuition payment for approved training seminars and workshops.
  • May provide on-call crisis services on a rotating basis with other staff.
  • Participate in treatment program cross-training, in order to assist with fulfilling other programmatic needs.
  • Ensure collateral involvement in treatment/safety planning.
  • Ensures essential services coverage for those needed for the OTP.
  • Ensure documentation of all treatment and clinically significant events in the Electronic Health Record as prescribed by CIHA policies and procedures and quality assurance standards.
  • Adhere to all applicable confidentiality policies, procedures, and practices for the maintenance and release of patient records.
  • Responsible for completing CIHA BH established forms needed, identifying and referring to community resources, CCA, and treatment planning provision and coordination of supportive services, and patient advocacy.
  • Assess and provide treatment for patients in individual and group modalities, exercising professional judgment and using a wide range of therapeutic skills to include individual and family counseling to assist patients and their families in dealing with chronic and acute behavioral health/substance use diagnoses.
  • Learn and effectively utilize the electronic health record and other software required for operation in the CIHA system.
  • Adhere to ASAM or other appropriate criteria when recommending and referring a client to the appropriate level of care
  • Assists with observed urine drug screens as outlined in Analenisgi's policies and procedures.
  • This position is expected to provide sufficient billable/reimbursable services to achieve the productivity standard established for this position.
  • May rotate with staff to provide walk-in access to care services daily, if needed. Triage clients to determine priority, emergent, urgent, routine, and work with the team to ensure the client is served in a timely manner.
  • Depending upon the location of services, maintain knowledge of all policies and procedures relevant to respective location; i.e., Cherokee Tribal Justice Center, Snowbird and Marble Clinics, CIHA inpatient, Analenisgi, Primary Care clinic.
  • Covers other CIHA-covered clinical locations when assigned, i.e. Kanvwotiyi.
  • Other duties as assigned.


Education/Experience/Qualifications

Required

  • Master’s degree in social work, or counseling required; or doctoral degree in psychology. Degree must be obtained from an accredited university that is accepted by appropriate licensing agencies in NC; and
  • Must be fully licensed in NC in a specific discipline, including Licensed Clinical Social Worker (LCSW), Licensed Clinical Mental Health Counselor (LCMHC), or Licensed Psychologist (PhD); and
    • For LCSW: Must have at least 2 years of full-time clinical social work experience after obtaining LCSW license (to allow for supervision of employees with LCSW-A)
    • For LCMHC: Must hold LCMHCS; or LCMHC with at least three years of professional counseling experience after obtaining LCMHC and formal clinical supervision training (to allow for supervision of employees with LCMHC-A)
  • Must be fully licensed as a Licensed Clinical Addictions Specialist (LCAS)
  • In addition to LCAS, must hold CSI or CCS credential by the North Carolina Addictions Specialist Professional Practice Board (NCASPPB); or must be willing to pursue CSI certification within first 6 months of employment (to allow for supervision of employees with LCAS-A, CADC, and CADC-I)
  • Must show competence in Behavioral Health and Substance Use Disorders by licensure, training, or experience; and
  • Leadership or supervisory experience demonstrated; and
  • Must be approved for and maintain medical credentialing, upon hire; and
  • Must possess valid driver's license.


Preferred

  • CCS Preferred
  • Specific experience working with Native Americans.
  • Experience in working in a medication-assisted treatment program.
  • Experience working in a healthcare setting.

  • Considerable knowledge of substance abuse to include diagnostic criteria for abuse, dependence, intoxication and withdrawal of all drug classifications, methods of evaluation and assessment, impact of mental illness on patient’s ability to negotiate treatment recommendations, current treatment approaches, modalities of care, and substance abuse and other programs or services available to patients.
  • Considerable skill in establishing rapport with a patient and in applying techniques of assessing psychological, behavioral, and psychosocial aspects of a patient’s problems.
  • Willingness to learn more about Cherokee culture/population and to have considerations for this when identified.
  • Ability to establish and maintain effective working relationships with members of the caseload and their families, other professional disciplines, and a variety of governmental and private resources and organizations in the community.
  • Ability to express ideas clearly and concisely and to plan and execute work effectively.


Complexity of Duties

This position is responsible for assisting in maximizing the available resources that best meet the constantly changing needs of the community. Must have the ability to interpret policy, laws, and regulations to make decisions when a precedent is not available. Duties require the application of clinical judgment and problem-solving skills to be effective. Judgment often involves decisions that may have life-or-death consequences. At times may be dealing with individuals who are highly agitated, emotional, or out of touch with reality.

Supervision Received

Works under the direct guidance of the Medication Assisted Treatment Manager, where goals, objectives, policies, and procedures are well defined. Works independently within applicable licensing regulations where latitude for creative thinking, initiative, and discretion is given to achieve department goals and objectives.

Responsibility for Accuracy

Most clinical errors would be detected with proofing and review procedures. Failure to properly manage and direct activities could result in ill will for the department, delays in patient treatment, and negative patient and community relations. Ineffective health planning could have adverse effects on the health status of the community by limiting access to necessary resources. The accuracy of data and research is critical in the allocation of tribal resources. Responsible for adherence to policy and procedure. Inappropriate actions or inaccuracies in judgment could result in improper diagnosis and treatment, serious liability for the tribe, or more serious health risks to patients.

Contacts with Others

Interacts frequently with coworkers, hospital staff, patients, stakeholders, and other health professionals for the purpose of exchanging information, obtaining, or providing assistance. Serves on various local, community, and health committees, boards, and advisory councils, as appropriate. Tact, courtesy, and professional conduct are required to maintain positive working relationships. Utmost sensitivity and confidentiality are required when dealing with patients and families.

Confidential Data

The provision of medical care requires that the incumbent maintain strict appropriate confidentiality regarding all patient information. The employee will have access to patient records and highly confidential information and must function with utmost integrity to safeguard this information. The employee complies with EBCI policy on confidentiality of patient records, Cherokee Indian Hospital Authority Confidentiality policy, HIPAA, and other federal law requirements. Violations of confidentiality regulations may result in disciplinary action and other actions as allowed by federal regulations.


Mental/Visual/Physical

Job duties can be mentally and emotionally stressful at times, given the complexity of the duties, management and supervision of staff, and the clinical areas being addressed with the people being served. Close mental concentration and attention to detail are required. Physical activities require the ability to be mobile, sit, reach, bend, and be in front of groups and deliver training for extended periods. Some physical exertion and stamina are required, such as those required to transport training materials or to travel. Must have visual acuity, manual dexterity, and the ability to speak and hear. Must be able to lift 30 lbs.

Environment

The employee works primarily in a hospital, inpatient, or outpatient clinical environment but may be required to work at other offices and locations. The work environment involves moderate risks of exposure to infectious disease, radiation, electrical hazards, and irritant chemical and explosive gases. Employees must strictly adhere to OSHA guidelines and protocol for maintaining sterile conditions.


Some travel is required. Infrequent overnight travel may be required for meetings or to attend training.


Emergency Privileges

In the case of an emergency, any member of the Medical Staff, to the degree permitted by his/her license, shall be permitted to do everything possible to save a patient’s life or save a patient from serious harm, regardless of the individual’s clinical privileges. This shall be defined as those situations in which a delay could result in serious harm or death and in which a more qualified member is not immediately available.
