The Hospice Social Worker provides emotional support, coordinates resources, and advocates for individuals facing life-limiting illnesses. Working under the supervision of an MSW and alongside the interdisciplinary team, the BSW helps patients and caregivers navigate the social, emotional, and practical challenges of end-of-life care.
As a Hospice Social Worker, you will:
Assess patient and family psychosocial needs, support systems, coping abilities, and barriers to care.
Collaborate with the supervising MSW and interdisciplinary team to develop and update individualized plans of care.
Provide supportive counseling within your scope of practice to help patients and families cope with illness, loss, and caregiving responsibilities.
Connect patients and families with community resources, financial assistance, transportation, and other support services.
Assist with advance care planning, advance directives, and funeral planning while respecting patient preferences.
Identify caregiver stress, safety concerns, and changes in family needs, and communicate concerns promptly.
Participate in interdisciplinary group meetings and coordinate services with nurses, physicians, chaplains, and other team members.
Support bereavement assessments and referrals in collaboration with the bereavement team.
Complete accurate, timely documentation according to agency guidelines.
Protect patient confidentiality and honor each family’s cultural, spiritual, and personal values.
Requirements
Bachelor of Social Work (BSW) from a program accredited by the Council on Social Work Education.
Current social work license or other credentials as required by the state of practice.
At least one year of social work experience in a healthcare setting.
Ability to provide services under the supervision of a qualified MSW.
Hospice, home health, or palliative care experience preferred.
Strong communication, organizational, and problem-solving skills.
Compassionate approach to working with patients experiencing serious illness and families facing grief and loss.
Valid driver’s license, reliable transportation, and current auto insurance for patient visits.
HomeCare HomeBase experience preferred.
