Minimum Qualifications:

  • Bachelor’s degree in Nursing, Healthcare Administration, Social Work, or a related field.
  • Minimum of 3 years experience in home health care, nursing, or patient care coordination.
  • Strong knowledge of home health care regulations, patient rights, and healthcare compliance standards.
  • Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare professionals.
  • Proficiency in electronic health records (EHR) systems and basic computer applications.

Preferred Qualifications:

  • Registered Nurse (RN) license or relevant professional certification in home health care.
  • Experience with case management or care coordination in a home health setting.
  • Familiarity with Medicare, Medicaid, and private insurance billing processes.
  • Training in patient education techniques and chronic disease management.
  • Bilingual abilities to serve diverse patient populations.

Responsibilities:

  • Conduct comprehensive assessments of patients' health status and home environments to identify care needs.
  • Develop and implement individualized care plans in collaboration with healthcare providers and family members.
  • Coordinate and monitor home health services, ensuring timely and appropriate delivery of care.
