Overview:

The purpose of mental health care coordination services is to engage priority individuals (recipients) and their natural supports who are not yet effectively connected with services to ensure service linkage, to enhance the delivery of treatment services and recovery supports and to improve outcomes among priority populations. Priority populations include: Children with SMI, SUD, or co-occurring disorders who demonstrate high utilization of acute care services, including CSU, inpatient and detox services. High utilization is defined as Children with 3 or more acute care admissions within 180 days; or, Children with acute care admissions that last 16 days or longer. The Care Coordinator will be responsible for coordination of services, supports, and cross system collaboration to ensure the individual’s needs are met.

 

ESSENTIAL/CORE JOB-SPECIFIC DUTIES AND RESPONSIBILITIES  



Utilize the LOCUS level of care tool and assessments to identify service needs and choices of the individual served


Serve as the single point of accountability for the coordination of an individual’s care with all involved parties (e.g. criminal justice, child welfare, primary care, housing, etc.). Coordinate care across systems Follow the individual from beginning to end until a warm-hand off is made


Engage the individual in their current setting (e.g., CSU, Detox, homeless shelter, client’s home, etc.)


Develop a client service plan with the individual based on shared decision making that emphasizes self-management, recovery and wellness, including transition to community based services or supports.


Meets Service Productivity standard of at least 75% (or 6 out of 8 work hours) for the provision of Care coordination services in accordance with applicable clinical/quality criteria which are supported by appropriate clinical record documentation which meet Client Record Handbook specifications, Avatar documentation requirements, and Agency business/clinical ethical standards.


Progress Notes:  Documents all services provided in the client record in accordance with Client Record Handbook Guidelines. Document must be finalized in Avatar within 48 hours from the time the service was rendered. Progress notes must be detailed and clearly reflect how the care coordinators efforts are linked to the services and goals in the client’s service plan and the client’s progress or lack of progress relative to the service plan. Maintain at least 85% compliance with progress note requirements on Clinical Reviews conducted during rating period.


Provide frequent contact for the first 30 days of services ranging from daily to a minimum of three times per week. Document all attempted contact to engage the individual in the client record.


QualificationsEducation/Experience: 


Bachelor's degree from an accredited university or college with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education, health science, or a related human services field.
One year full time or equivalent experience working with Children experiencing serious mental illness preferred.
Other Bachelor’s degree from an accredited university or college with three years full-time or equivalent experience working with Children experiencing serious mental illness preferred.  


 

Licensure or Certification:


Current valid driver’s license.
Maintain Apalachee Designated Auto Driver status.
