Salary Range: $70,304 - $78,000 annually, DOE

What You'll Do: The Impact You'll Make
As a Clinical Case Manager, you will provide direct clinical services and care coordination to individuals residing in interim housing, supporting their transition into stable community living.

Clinical Assessment and Treatment Planning
• Conduct comprehensive intake assessments and develop individualized, client-centered treatment plans.
• Monitor client progress and adjust treatment interventions as needed.
• Facilitate individual counseling and psychoeducational group sessions.

Therapeutic Services and Client Support
• Deliver evidence-based interventions grounded in trauma-informed care and harm reduction principles.
• Provide one-on-one counseling to increase client insight, motivation, and readiness for change.
• Support clients in building recovery skills and reducing isolation.

Care Coordination and Crisis Intervention
• Coordinate referrals to medical, mental health, and substance use services.
• Connect clients to community-based resources, housing pathways, and support systems.
• Provide crisis intervention and respond to urgent client needs across housing sites and in the field.

Collaboration, Documentation, and Compliance
• Participate in multidisciplinary team meetings, case conferences, and partner coordination.
• Maintain timely and accurate documentation in electronic health records.
• Ensure compliance with agency policies, funder requirements, and regulatory standards.

What You'll Bring: Your Qualifications

Education and Experience
• Master’s degree in Social Work, Marriage and Family Therapy, Counseling, or related field required.
• Active BBS registration (ASW, AMFT, or APCC) required within 90 days of hire.
• Experience providing clinical services in community mental health, substance use treatment, or correctional settings preferred.
• Familiarity with trauma-informed care, harm reduction, and motivational interviewing strongly preferred.
• Military-experienced candidates are encouraged to apply.

Skills and Abilities
• Strong clinical assessment, documentation, and crisis intervention skills.
• Excellent communication (written and verbal), organizational, and interpersonal abilities.
• Ability to manage multiple priorities and work effectively in a fast-paced environment.
• Ability to work both independently and collaboratively.
• Bilingual in Spanish and English preferred.

Additional Requirements
• Valid California Driver’s License, clean driving record, and insurability required.
• Ability to travel between program sites.
• Live Scan clearance required.
• COVID-19 vaccination with at least one booster required (approved exemptions accommodated).
