  • Job Scope: Completes LPHA review of IM-CANS/Assessments for Behavioral Health Divisions. Provides feedback to respective staff members regarding edits and changes that need to be made. Follows up with staff to ensure changes are made and signs assessments in a timely manner. Provide back-up/on-call coverage for Front Door mental health assessments.
 
Clinical Responsibilities
  • Complete IM-CANS review acting as LPHA for staff on Behavioral Health
  • Division; providing feedback and coordinating with assessor as needed (LPHA candidates only)
  • Provide back-up, on-call to the Front Door assessment team; respond to referrals by reaching out to hospitals and completing assessment within 24 hours of referral to meet grant requirements
  • Complete comprehensive strengths-based, client-centered, and trauma-informed clinical mental health assessments to adults at local hospitals
  • Administrative and Performance Requirements
  • Complete tracking and update CIS and spreadsheets to identify assigned IM-CANS for review
  • Ensure follow-up with assessor so that IM-CANS are signed and completed within 14 days

  • ssor’s supervisor/manager if needed, to provide feedback on assessments and/or timeliness of updates/completion
  • Complete all clinical documentation and required reports in an accurate and timely manner; including referrals, desk reviews, IM-CANS, and other assessment tools
  • Participate in clinical and administrative supervision as directed, to receive and offer feedback and for performance evaluation and seek additional clinical and program consultation as needed
  • Participate in All Staff meetings & other staff meetings and trainings as directed
  • Interpret and communicate, at the service delivery level, Center policies and procedures
  • Meet agency quality and compliance standards
 
Education, Experience and Other Requirements: 
  • Masters or doctorate degree from an accredited college or university of social work, psychology, or a related discipline.
  • Illinois LCPC/LCSW or equivalent that meets IL Rule 132/140 LPHA designation
  • Experience in assessment for individuals with mental health needs; at least 1 year experience completing IM-CANS assessments preferred
 
  • Must obtain IM-CANS certification and demonstrate proficiency within 45 days
  • Basic computer proficiency, knowledge of Word and internet skills Basic technology, smartphone, and computer proficiency; knowledge of web-based team communication tools (ex. Microsoft Teams), electronic health records, telehealth programs, and internet skills
Schedule/Timekeeping
  • Non-Exempt, Part-Time , Hourly, Up to 20 hours week
