Part Time Children’s Case Manager – Respite & After-School Program

Our programs serve children with diverse needs, providing structured activities, skill-building opportunities, and compassionate support for families. We’re seeking a Children’s Case Manager to coordinate services, ensure quality care, and help each child thrive.

Position Summary

The Children’s Case Manager is the primary liaison between families, program staff, and community resources. This role ensures that each child’s needs are met through individualized planning, consistent communication, and ongoing monitoring of services in our respite and after-school programs.

Key Responsibilities

  • Conduct intake assessments and develop Individualized Service Plans (ISPs) in collaboration with families and staff.
  • Coordinate daily and long-term supports for children in respite and after-school settings.
  • Monitor program participation, progress, and satisfaction, making adjustments as needed.
  • Maintain accurate, timely documentation in compliance with agency and regulatory standards.
  • Serve as an advocate for children and families, connecting them to community resources.
  • Collaborate with program staff to ensure activities are inclusive, engaging, and developmentally appropriate.

Qualifications

  • Available 20 hours per week
  • Bachelor’s degree in Social Work, Psychology, Education, or related field (Master’s preferred).
  • Minimum of 1–2 years working with children in case management, education, or human services.
  • Experience with intellectual/developmental disabilities
  • Understanding of NJ Children's System of Care and experience with PerformCare
  • Strong organizational and communication skills.
  • Ability to build trust with children, families, and staff.
  • Knowledge of child development and community resources.
