PURPOSE OF POSITION:

Responsible for overseeing all case work services. Ensures the completion of all required paperwork and full compliance with all City and State RHY regulations

and standards.

REPORTS TO:

            Program Supervisor

 

SPECIFIC RESPONSIBILITIES:

  • Complete Initial Service Plan Review & Case Conferences for clients/families & facilitate additional family meetings and/or program visits.
  • Coordinate services identified in the Initial Service Plan & subsequent reviews to assist clients in meeting their short & long-term goals.
  • Complete home assessments for clients who will be utilizing weekend passes.
  • Report any suspected abuse or neglect to Justice Center/State Central Register.
  • Become DYCD Connect trained & proficient.
  • Oversee Counseling Specialists regarding their specific responsibilities supporting case management services.
  • Provide aftercare services to clients for 90-days post discharge.
  • Maintain case records & ensure necessary documents are present on all current residents in their 90-day aftercare period.
  • Facilitate weekly force field/team meeting.
  • Conducts initial intake interviews of prospective residents in the absence of Intake Coordinator.
  • Work collaboratively with Program Coordinator, Counseling Specialists and Intake Coordinator to ensure quality of service delivery.
  • Ability to work efficiently and independently.
  • Highly organized
  • Attend & Participate in QI/QA & program admin meetings

 

QUALIFICATIONS:

  • Bachelor degree in a related field is required
  • MSW preferred.

WORKING CONDITIONS

  • Working conditions vary between office work and field visits.
