Job Summary: The Housing Solutions Case Manager, under the direction of the Housing Solutions Manager, assists Mercy House clients in obtaining and sustaining permanent housing through the Housing First Model. The position provides case management and housing advocacy to clients entering the system of care, assesses client needs, evaluates housing plans, and tracks progress toward housing goals. The Case Manager advocates for and supports clients enrolled in the Rapid Re-Housing and Homeless Prevention Programs.

Essential Duties and Responsibilities:
Program Oversight


Attend Family Support Team (FST), multi-disciplinary team (MDT), and other meetings as assigned
Provide comprehensive case management services, including intakes, housing stabilization plans, goal setting, referrals, and ongoing monitoring.
Use client-centered, harm reduction, Trauma-Informed Care, and Motivational Interviewing approaches to support housing stability
Maintain accurate case notes using the GIRP (Goal, Intervention, Response, Plan) format and keep client files current
Develop individualized case management plans, including financial and housing goals, with clients and families
Meet regularly with clients to monitor progress, address barriers, and ensure program requirements are met
Provide tenant education, advocacy, referrals, and connections to community resources
Empower clients by building on their strengths and addressing needs in a respectful, objective, and supportive manner
Maintain accurate and timely client information in HMIS and update program master logs daily
Conduct home visits and Habitability Inspections as needed
Assist clients with employment and housing-related barriers
Maintain knowledge of community resources and connect clients to appropriate services
Follow all agency case management policies and maintain an assigned caseload
Participate in weekly Coordinated Entry System (CES) meetings, following all policies and procedures to request client matches and submit updates
Assist clients with housing searches, provide housing leads, and facilitate relationships between clients and landlords
Support and mediate landlord/resident issues to help clients maintain housing
Complete assistance recertifications, gather required documentation, and conduct client follow-ups as assigned
Understand and prioritize quality outcomes for clients


Team Tasks


Collaborate with staff, interns, volunteers, leasing agents, and other departments to support the agency’s mission and client housing goals
Attend and participate in staff, supervision, community, training, and monthly Housing Solutions team meetings
Discuss client progress and needs, share resources, and coordinate services with the Housing Solutions team
Foster positive working relationships with case managers, leadership, and community partners
Work effectively in a fast-paced environment; multitask, adapt to changing workloads, and demonstrate reliability, trustworthiness, and empathy


Administration


Maintain complete and accurate client documentation, including stabilization plans, case notes, call logs, emergency plans, and related records
Submit and upload supporting documentation related to client services in a timely manner
Assist with program activities, data collection, and other administrative needs
Develop agency partnerships to enhance available services
Maintain confidentiality and security of case files and client information and maintain knowledge of applicable HIPAA requirements
Ensure documents submitted on behalf of clients are accurate, valid, and timely
Utilize computers and appropriate software effectively
Respond to calls and emails promptly
Work independently while effectively prioritizing tasks and deadlines
Occasionally lift items up to 25 lbs. and work for extended periods sitting at a desk and using a computer
Complete other projects and duties as assigned by the Housing Solutions Supervisor and/or Manager

Qualifications: Requirements:
Education and Experience


Bachelor’s degree in social work or a related field and/or four years of experience in social work or human services within a public or private organization required
Experience working with individuals experiencing homelessness highly preferred


General


Valid California driver’s license, proof of insurance, and reliable vehicle required. Must be comfortable using a personal vehicle to conduct home visits, visit clients at shelters, meet clients in the community, and perform other job-related travel
Ability to work effectively with diverse populations; demonstrate ethical behavior; plan, organize, and prioritize duties; provide crisis intervention as needed; communicate clearly and professionally, both verbally and in writing; maintain a positive, professional, and safe work environment; and establish effective working relationships with clients, landlords, social workers, and other service providers
Ability to work in a fast-paced environment while maintaining confidentiality of sensitive client information
Demonstrated dependability, responsibility, and effective, respectful communication skills
Proficiency in Microsoft Office and basic computer skills, including typing, email, and internet use
Ability to work occasional weekends and/or evenings to respond to crisis situations as needed
Fluency in Arabic, Farsi, Spanish, or Vietnamese is highly valued
