Work Schedule: Monday - Friday, 8:30 am - 5:00 pm

Hampton-Newport News Community Services Board is seeking a full-time Substance Use Disorder (SUD) Case Manager for Project Link in Hampton, VA. This position is ideal for social workers, counselors, case managers, and other human services professionals interested in supporting women, children, and families affected by substance use and co-occurring disorders.

Your day as a Case Manager for Project Link

As a Case Manager for Project Link, you will provide case management services to women experiencing substance use and co-occurring disorders, along with their children from birth through age seven who may be at risk for emotional or behavioral challenges.

Using a person-centered, trauma-informed, and holistic approach, you will help families build stability, improve wellness, and access services that support long-term success.

Responsibilities include:

  • Conduct outreach, assess individual needs, and eligibility for services
  • Develop treatment plans and monitor progress
  • Provide case management, crisis intervention, and supportive counseling
  • Coordinate behavioral health, medical, housing, and community services
  • Connect individuals and families with resources and supports
  • Advocate for individuals and collaborate with providers and community partners
  • Complete documentation, case notes, and reports
  • Participate in individual review meetings, discharge evaluations, and planning
  • Conduct on-site drug screenings for individuals with substance use disorders

Requirements

  • Bachelor’s degree in Human Services (Social Work, Psychology, Sociology or Counseling)
  • One (1) year experience providing case management services to individuals with substance use disorders or co-occurring mental health and substance use disorder
  • Valid Virginia Driver's License
