Position Summary

Interdynamics, Inc. is seeking a dedicated, compassionate, and highly organized Case Manager to support the delivery of Outpatient Treatment Services (OTS) under its contract with the Court Services and Offender Supervision Agency (CSOSA).

The Case Manager serves as a primary coordinator of care for participants enrolled in OTS and is responsible for facilitating access to comprehensive, person-centered services that promote recovery, stability, treatment engagement, and successful community reintegration. The Case Manager works collaboratively with participants, counselors, clinical leadership, Community Supervision Officers (CSOs), and community-based service providers to ensure continuity and coordination of care.

Key responsibilities include participant intake and engagement, care coordination, development and monitoring of Individualized Care Plans (ICPs), treatment and service documentation, participant monitoring, referrals, transition planning, and discharge/aftercare coordination.

The ideal candidate has experience in behavioral health, case management, care coordination, or a related human services field; a strong understanding of substance use disorders and recovery principles; and the ability to work effectively with justice-involved individuals in a professional, respectful, and trauma-informed manner.

Essential Duties and Responsibilities

1. Participant Intake and Engagement

  • Conduct intake interviews with participants within seven (7) days of receipt of a CSOSA Billing Authorization.
  • Coordinate participant admissions and facilitate timely enrollment into appropriate treatment services.
  • Conduct program orientations and clearly explain participant rights, responsibilities, treatment expectations, and program requirements.
  • Provide participants with program handbooks, schedules, contact information, and other relevant program materials.
  • Obtain, verify, and maintain required Releases of Information (ROIs), consent forms, and confidentiality documentation.
  • Notify Community Supervision Officers (CSOs) of participant admissions and provide required updates regarding attendance and engagement.
  • Establish and maintain a professional, supportive relationship with each participant.
  • Assist participants in understanding treatment requirements, available resources, and the services necessary to support successful treatment engagement.

2. Care Coordination and Case Management

  • Coordinate behavioral health, substance use treatment, medical, housing, employment, educational, vocational, and other supportive services based on participant needs.
  • Facilitate referrals to internal programs and external community-based service providers.
  • Monitor participant engagement and identify barriers that may interfere with treatment participation or successful community reintegration.
  • Assist participants with applications, eligibility requirements, documentation, and access to community resources and benefits.
  • Coordinate appointments, referrals, follow-up services, and other supports to promote continuity of care.
  • Maintain regular communication with participants and service providers regarding service needs, referrals, and outcomes, consistent with applicable confidentiality requirements.
  • Participate in interdisciplinary team meetings, case conferences, and staffing sessions.
  • Collaborate with counselors, Clinical Supervisors, CSOSA personnel, and other stakeholders to promote coordinated and comprehensive care.
  • Advocate for participant access to appropriate services while maintaining professional boundaries and program requirements.

3. Individualized Care Planning

  • Develop Individualized Care Plans (ICPs) based on assessment findings, identified needs, participant strengths, preferences, and recovery goals.
  • Engage participants in establishing realistic, measurable, and person-centered treatment and recovery goals.
  • Identify strengths, barriers, risks, and service needs that may affect treatment participation and community stability.
  • Document treatment objectives, interventions, recommended services, and anticipated service schedules.
  • Ensure ICPs are person-centered, clinically appropriate, medically necessary, and consistent with applicable ASAM Criteria and program requirements.
  • Monitor participant progress toward established goals and revise care plans when needs, circumstances, or treatment priorities change.
  • Update ICPs following significant changes in participant circumstances, treatment status, or identified service needs.
  • Coordinate with the Clinical Supervisor to obtain required reviews, approvals, and signatures for treatment plans and related documentation.

4. Assessment and Documentation

  • Assist in coordinating behavioral health and substance use assessments, reassessments, and ongoing evaluations.
  • Ensure required clinical and case management documentation is accurate, timely, complete, and consistent with program standards.
  • Maintain complete, organized, and current electronic treatment records.
  • Document participant contacts, encounters, interventions, referrals, care coordination activities, and follow-up efforts.
  • Maintain documentation in accordance with applicable federal and local requirements, CSOSA contractual requirements, and Interdynamics, Inc. policies and procedures.
  • Protect participant privacy and maintain confidentiality in accordance with HIPAA, 42 CFR Part 2, and other applicable privacy and confidentiality requirements.
  • Ensure documentation supports continuity of care, service coordination, quality assurance, and program compliance.

5. Participant Monitoring, Support, and Advocacy

  • Monitor participant attendance, engagement, progress, and compliance with established treatment requirements.
  • Follow up promptly with participants after unexcused absences or other indicators of disengagement and implement appropriate re-engagement strategies.
  • Identify emerging participant needs, barriers, or concerns and coordinate appropriate interventions and referrals.
  • Support participants in developing practical life skills, recovery supports, and strategies for maintaining stability in the community.
  • Provide ongoing encouragement, education, resource coordination, and advocacy throughout the participant's treatment episode.
  • Assist participants in navigating systems and services that may affect housing, employment, healthcare, education, transportation, and other areas of stability.
  • Support participants during transitions between levels of care and service providers.
  • Communicate significant concerns or changes in participant status to the appropriate clinical and supervisory personnel in accordance with program procedures.

6. Discharge Planning and Aftercare

  • Begin discharge planning early in the participant's treatment episode and incorporate discharge goals into the Individualized Care Plan.
  • Collaborate with participants, counselors, Clinical Supervisors, CSOs, and community providers to develop appropriate discharge and aftercare plans.
  • Coordinate referrals to continuing treatment, recovery support, healthcare, housing, employment, educational, and other community-based services.
  • Assist participants with scheduling follow-up appointments and establishing connections with ongoing support services prior to discharge.
  • Confirm, when appropriate and permitted, that participants have been connected to identified aftercare resources.
  • Document discharge planning activities, referrals, participant progress, and final disposition in accordance with program requirements.
  • Promote continuity of care and reduce gaps in services during transitions from OTS to community-based or other continuing care.

7. Communication and Collaboration

  • Maintain professional and timely communication with participants, CSOs, clinical staff, supervisors, referral sources, community providers, and other authorized stakeholders.
  • Communicate participant progress, service needs, barriers, and significant changes in status in accordance with applicable policies and confidentiality requirements.
  • Participate in case conferences, interdisciplinary meetings, quality improvement activities, and other meetings as assigned.
  • Develop and maintain effective working relationships with community partners and service providers.
  • Represent Interdynamics, Inc. professionally when interacting with participants, government agencies, referral sources, and community organizations.

8. Compliance and Quality Assurance

  • Comply with all applicable federal, state, and local laws and regulations governing behavioral health and substance use treatment services.
  • Adhere to CSOSA contract requirements, Interdynamics, Inc. policies and procedures, and applicable clinical and documentation standards.
  • Maintain professional and ethical standards when working with justice-involved populations.
  • Complete required documentation and administrative tasks within established timelines.
  • Participate in quality assurance, audits, performance improvement activities, and corrective action processes as assigned.
  • Maintain required training, credentials, and professional development activities.
  • Perform other duties and responsibilities consistent with the position and assigned by the supervisor.

Qualifications and Competencies

Required Qualifications

  • Bachelor's degree in social work, psychology, counseling, behavioral health, human services, or a related field, or equivalent education and relevant experience as permitted by applicable program requirements.
  • Experience providing case management, care coordination, behavioral health, substance use, or related human services.
  • Knowledge of substance use disorders, recovery-oriented systems of care, and community-based behavioral health services.
  • Understanding of the needs and challenges faced by justice-involved individuals.
  • Strong organizational, documentation, communication, and time-management skills.
  • Ability to establish effective professional relationships with participants and multidisciplinary teams.
  • Ability to maintain confidentiality and exercise sound professional judgment.
  • Proficiency with electronic health records and standard computer applications.
  • Ability to work independently while also functioning effectively as part of an interdisciplinary team.

Preferred Qualifications

  • Experience working with individuals involved in the criminal justice system or under community supervision.
  • Experience working within outpatient substance use or behavioral health treatment programs.
  • Familiarity with ASAM Criteria and evidence-based practices for substance use disorders.
  • Experience coordinating services across multiple systems, including behavioral health, healthcare, housing, employment, and social services.
  • Knowledge of HIPAA, 42 CFR Part 2, and CSOSA-related requirements.
  • Experience working with electronic health records and other clinical documentation systems.

Work Environment

This position operates in a behavioral health and community-based treatment environment and requires regular interaction with participants, clinical staff, CSOSA personnel, and community service providers. The Case Manager must be able to manage competing priorities, maintain accurate documentation, and respond appropriately to changing participant needs while adhering to all applicable safety, confidentiality, contractual, and organizational requirements.
