Position Summary:

Unison Health is seeking a Case Manager to support children and adolescents receiving care in our inpatient Children's Wellness Center (CWC) program. This role coordinates individualized treatment and discharge planning, collaborates with the interdisciplinary treatment team, connects families with community resources, and advocates for each child's ongoing behavioral health and recovery needs to promote a safe and successful transition of care.

Key Responsibilities & Role Highlights:

  • Engage clients in developing daily living, recreational, and independent living skills
  • Monitor clients’ needs through observation, check-ins, and ongoing communication
  • Implement individualized treatment/service plans (ITP/ISP) and report changes or concerns
  • Support symptom management, coping skills development, and community integration
  • Assist clients in accessing community resources, healthcare providers, and benefits
  • Provide advocacy, outreach, and re-engagement support for missed appointments or disengagement
  • Respond to and help stabilize crisis situations using appropriate interventions
  • Maintain accurate, timely documentation in the electronic health record, including assessments, progress notes, and treatment plan updates
  • Participate in team meetings, supervision, training, and collaborate with clinical and support staff to ensure coordinated care

Education & Experience Requirements: 

  • Minimum of a bachelor’s degree in social work, Counseling, Psychology, or a related field

  • Must meet Ohio MHAS criteria for Qualified Mental Health Specialist (QMHS) if not licensed

  • Valid driver’s license and reliable transportation required

  • Must be insurable under the agency’s commercial liability policy

  • Experience working with children and families preferred

  • Must demonstrate knowledge in:

    • Mental illness and treatment

    • Individual and group counseling

    • Behavioral health assessments and crisis intervention
