Join our dynamic team at Quadrant Health Group! QHG delivers reliable, process-driven support to behavioral health programs. We’re hiring a Case Manager to coordinate care across our Greater Texas Behavioral Clinic facility (located in TX) and our new facilities Des Moines Wellness (located in IA) and Wellness Ranch Kentucky’s Virtual SUD/MH IOP/OP program. This role is ideal for someone who thrives in a fast-paced, structured environment and can balance client engagement with precise documentation and follow-through.  



What You'll Do

 

  • Coordinate care across programs: Serve as a central point of contact for clients across both facilities, ensuring seamless communication, scheduling, and service delivery.  
  • Lead and facilitate groups: Conduct psychoeducational and support groups (group facilitation experience strongly preferred).  
  • Client engagement & assessment: Communicate regularly with clients to assess needs, identify barriers, and connect them with appropriate services.  
  • Provider coordination: Schedule and coordinate provider visits on behalf of clients, ensuring continuity of care.  
  • Documentation is critical: Maintain timely, accurate case management notes, releases, contacts, and all required documentation in the EMR (Kipu).  
  • External communication: Submit updates and required documentation to probation/parole officers, legal entities, employers, or other stakeholders as needed.  
  • Support transitions: Facilitate admissions, ongoing engagement, and discharge planning with strong aftercare coordination (provide recommendations for primary care, psychiatry, outpatient therapy, peer support, etc.). 
  • Monitor outcomes & compliance: Track engagement, attendance, no-shows, authorizations, and overall client progress.  
  • Escalate appropriately: Identify risks or barriers and escalate to the proper clinical or administrative channels in a timely manner.  

Requirements

Experience

  • 1-3+ years in behavioral health case management or care coordination (SUD/MH populations preferred)
  • Experience facilitating groups strongly preferred
  • Familiarity with payer processes, compliance expectations, and community resources

Education/Training

  • (Preferred) Bachelor's degree in social work, human services, psychology, or related field - or equivalent experience
  • Relevant certifications (case management, peer/recovery, care coordination) are a plus

Schedule

*PLEASE NOTE: THIS POSITION REQUIRES WORKING UNTIL 9:00 PM (EST) ON TWO PREDETERMINED DAYS EACH WEEK. APPLICANTS MUST BE ABLE TO ACCOMMODATE THIS SCHEDULE.

As part of this role, employees will facilitate virtual outpatient group sessions held Monday through Thursday. OP groups run from 7:00–9:00 PM EST, while IOP groups run from 7:00–10:00 PM EST. Following successful training and acclimation, you will be assigned two to three group sessions per week in coordination with your team.  

On your assigned group facilitation days, your schedule will shift to either 1:00–9:00 PM EST or 2:00–10:00 PM EST, depending on the facility and program to which you are assigned. This adjusted schedule accommodates evening programming. Your remaining workdays will generally follow the standard 9:00 AM–5:00 PM EST schedule.  

Candidates should be comfortable working this adjusted schedule and supporting the needs of our programs and clients.
