Job DetailsJob Location: Worthington, MN 56187Position Type: Full TimeSalary Range: $22.00 - $32.00 HourlyJob Shift: AnyElevaCare is seeking a compassionate Behavioral Health Practitioner to provide support and guidance for residents at our Intensive Residential Treatment Facility, Unity House. In this role, you will help residents meet their physical, emotional, and therapeutic needs by implementing treatment plans, advocating for residents, facilitating group activities, assisting with appointments and transportation, administering medications, supporting meals, and documenting progress. You will collaborate closely with the treatment director, clinical supervisors, social workers, nurses, and other team members to maintain a safe, supportive, and therapeutic environment. Flexibility to work evenings, nights, weekends, and holidays is required.


EDUCATION, EXPERIENCE, AND SKILLS REQUIRED
Candidates must meet one of the following qualifications:

Bachelor’s degree path: Bachelor’s in behavioral sciences or related field, 2,000 hours clinically supervised experience, completion of required training, and practicum/internship experience.

Advanced experience path: 4,000 hours experience working with mental health, substance use, TBI, or developmental disability clients, completion of required training, or ongoing supervision until requirements are met.

Graduate level: Master’s or other graduate degree in behavioral sciences or related field, or enrollment as a graduate student assigned to ElevaCare for clinical training.

KEY RESPONSIBILITIES


Implement and enforce residential program policies and procedures
Collaborate with team members to develop, review, and carry out individualized treatment plans
Provide direct support to residents: counseling, advocacy, appointments, transportation, meals, medication administration, and group activities
Maintain accurate and timely documentation of treatment, progress, and evaluations
Participate in supervision, staff meetings, and trainings
Maintain relationships with referring agencies, guardians, and programs involved in residents’ lives
Support crisis intervention as needed



PHYSICAL AND ENVIRONMENTAL REQUIREMENTS


Regularly stand, walk, talk, hear, and use hands; lift/move up to 50 lbs
Ambulate within and between departments; stoop, kneel, crouch, crawl, sweep, mop, shovel
Visual abilities include close, distance, color, peripheral vision, depth perception, and focus adjustment
Exposure to healthcare environments, moderate noise, wet/humid conditions, and occasional outdoor settings



SKILLS AND QUALIFICATIONS


Strong communication, critical thinking, problem-solving, and multi-tasking skills
Proficiency with computers (Windows environment)
