Work Schedule & Location: This is an in-office position, with flexibility for occasional remote work or adjustments to daily schedule as operational needs allow. Core working hours fall between 8:00 a.m. and 6:00 p.m., with flexibility in start and end times.

Position Overview:
The Case Manager II - Eviction Prevention provides specialized rent-related housing stabilization support to Minneapolis Public Housing Authority (MPHA) residents across Agate Housing and Services' Housing Stability program. The position leads direct assistance with past-due rent, repayment agreements, and related financial barriers to continued tenancy, coordinating with residents, MPHA property management, assigned Case Managers, and Program Managers to identify practical steps toward housing retention.
The position also supports staff in initiating and implementing housing loss prevention plans and maintains centralized tracking of all plans across MPHA properties, including actions, deadlines, follow-up, and housing retention or transition outcomes. It assists with failed inspection follow-up and voluntary mutual lease terminations when appropriate, including resident-centered transition planning and connection to alternative housing resources. Assigned Case Managers retain their ongoing resident relationships; this position provides direct intervention for selected situations rather than taking over every housing loss prevention plan.
Position Responsibilities:
- Lead specialized rent-related eviction prevention services across MPHA properties, including outreach and intervention with residents who have past-due rent, missed payments, or repayment agreement concerns.
- Engage residents to understand the circumstances affecting rent payment and housing stability, identify immediate deadlines and barriers, and develop practical next steps with the resident.
- Assist residents and MPHA property management in developing realistic repayment plans for past-due rent, subject to MPHA approval, and identify financial assistance and income-related resources that may support the agreement.
- Follow up directly on assigned rent-related cases, including repayment plan progress, upcoming payments, changes in income, and barriers that could prevent residents from meeting agreed-upon terms.
- Support assigned AMP Case Managers and Program Managers with failed inspection follow-up and other tenancy concerns when they are part of a housing loss plan or require additional eviction prevention support.
- Help initiate and develop individualized housing loss plans with residents and assigned staff; clarify the housing concern, action steps, responsible parties, deadlines, needed resources, and follow-up expectations.
- Provide consultation and coordination to AMP Case Managers and Program Managers implementing housing loss plans; help align resident, MPHA property management, and community provider responsibilities without assuming primary case management for every plan.
- Maintain centralized tracking of all housing loss plans across the MPHA portfolio, including plan status, responsible staff, key deadlines, progress, and outcomes such as housing retained, voluntary mutual lease termination, transition to alternative housing, or eviction; obtain updates from assigned teams.
- Review the housing loss plan tracker regularly with Program Managers and the Program Manager - Specialized Housing Stability, surface emerging patterns and time-sensitive concerns, and help teams adjust or escalate interventions when needed.
- Help residents understand rent-related notices, lease expectations, and available choices; connect residents to tenant advocacy or legal assistance when guidance on rights or lease agreements is needed.
- When housing retention is not feasible, assist residents who are considering voluntary mutual lease termination by coordinating with MPHA property management, AMP Case Managers, and community partners; support informed resident choice, housing transition planning, referrals, and timely follow-up without providing legal advice or pressuring residents to terminate their lease.
- Coordinate with Agate benefits and eligibility staff regarding income changes, benefits interruptions, and recertification matters that affect rent or housing stability, without duplicating specialized services.
- Support residents and AMP staff in accessing rental assistance, benefits, mediation, community resources, and other services relevant to housing loss plan action steps.
- Use trauma-informed, person-centered, and strengths-based approaches in resident outreach, problem solving, and conflict resolution.
- Provide targeted direct resident support at MPHA properties through scheduled meetings, home visits, and community-based outreach when rent-related needs or selected housing loss plans warrant involvement.
- Consult with the Program Manager - Specialized Housing Stability and coordinate with the Crisis & Float Team or emergency and specialized services when a resident situation requires additional support.
- Document direct service work and maintain accurate, timely portfolio-wide housing loss plan tracking; communicate plan updates and documentation expectations to assigned AMP staff.
- Participate in case consultations and team meetings and support improvements to MPHA-wide processes for initiating, implementing, tracking, and closing housing loss plans.
- Manage time-sensitive rent-related referrals alongside portfolio-wide tracking and staff consultation; communicate capacity constraints and cases requiring management attention.
- Perform other related duties as assigned.
Required Qualifications:
- Bachelor's degree or minimum two years of experience in a similar case management, housing, or human services role.
- Demonstrated experience supporting individuals or families with complex service needs, housing instability, or crisis intervention.
- Ability to build effective relationships with residents of diverse ages, backgrounds, household compositions, and service needs.
- Strong problem-solving, communication, advocacy, crisis intervention, and conflict-resolution skills.
- Ability to independently assess situations, prioritize competing needs, and coordinate appropriate interventions while collaborating with a multidisciplinary team.
- Ability to establish and maintain effective working relationships with property management, community providers, and other service partners.
- Strong organizational skills and ability to maintain accurate, timely case management documentation.
- Proficiency with Microsoft Office products and ability to learn required data and case management systems.
- Ability to travel to assigned MPHA properties and community locations as needed; must meet applicable driving and background screening requirements for the assignment.
- Must have valid MN Driver's License and pass background check.β―β―
- Demonstrated ability to network via email, telephone, and in person to advocate for participants.β―
- This position requires use of your own personal vehicle and will require insurance liability coverage of at least one hundred thousand dollars per person and three hundred thousand dollars per occurrence and must have a valid Driver's License and pass a driving record background check.
Preferred Qualifications:
- Experience in public housing, resident services, eviction prevention, housing stabilization, or related community-based services.
- Experience providing crisis intervention, de-escalation, and short-term intensive case management to individuals or families with complex service needs.
- Experience working with older adults, people with disabilities, families with dependent children, or residents with complex service needs.
- Experience providing mobile or community-based case management services across multiple locations.
- Experience coordinating with property managers, community providers, and interdisciplinary teams to address housing stability concerns.
- Familiarity with community resources relevant to the needs of MPHA residents.
- Knowledge of Housing First, Harm Reduction, and trauma-informed care principles.
- Licensed Social Worker (LSW) in Minnesota.
- Fluency in languages used by residents, including Spanish, Amharic, Oromo, Somali, or American Sign Language (ASL).
