The Case Manager is responsible for overseeing the daily operation of assigned homes, including management of all Team Members, ensuring the health and safety needs of Members are met; and acting as an Advocate on behalf of New Vista Members.

 

 

Essential Job Functions:

 

 

Oversight of Member Services:

 


Creation and/or implementation of each Member’s Person-Centered Plan and Habilitation Plan daily


To include all service needs including but not limited to: medical, employment, daily living skills, assistive devices/supports, activities, health needs, specialized services, and mental health.
Active monitoring of the implementation of support plans; assessment of progress; and effectiveness of supports provided to individuals;





Review residential support management logs, 24-hour logs, monthly progress notes
Oversee and train Senior LSI and team members regarding the recommendations obtained during PCP, HRC-BIC meetings
Available by phone and in person to assist and participate in emergency events, such as unplanned hospitalizations and/or unscheduled meetings or Guardian requests
Required to assist in covering shifts in Homes as needed


 

Supervision of Team Members:

 


Addresses concerns immediately, such as inappropriate interactions between co-workers and Members and/or between Members living together, including interactions that may rise to the level of Mistreatment (abuse, neglect, exploitation)
Create and issue corrective action in cases of policy and procedure violations
Report allegations of Mistreatment (abuse, neglect, exploitation) misappropriation of funds, and fraudulent activity to QA/Compliance Manager and Case Manager Supervisor.
Oversight of/and accurate and timely completion of billing packets, incident reports, and any/all documentation as required by New Vista, DRC, HRC-BIC or other providers (social security, etc.)

QualificationsSkills and Abilities:

 


Verbal and written communication skills including the ability to write and provide documentation as described
Read and understand client information, instructions, case notes, incident reports, manuals 
Visually observe, hear, and respond to Individuals
Complete New Vista new employee training (CPR, medication management (MEDS), behavior management (Safety Care)
Maintain annual training as required and scheduled


 

 

 

 

 

 

Education and Experience:

 


Minimum of five years experience in a related field
Bachelor’s degree in a field related to Human Services; Preferred
Pass all pre-employment and post-employment screening such as local, State, and FBI


background checks; sexual offender screening

 

Physical Demands:


Required to stand, use hands to handle or feel objects, tools or controls, frequently talk or hear, reach with hands, lift arms above and below shoulder level, required to sit for extended periods, occasionally stoop, kneel, or crouch during regular work activities



Ability to lift and/or move up to 25 pounds
