Job Summary:

As a Health Home Care Manager your function is to act as a leader in providing appropriate Health Home services to approximately 50-60 active clients according to the guidelines set out in the ACCESS Program Manual (and as required by the NYSDOH – HH, AIDS Institute).

 

Primary Duties & Responsibilities include, but are not limited to:

  • Be familiar with the ACCESS Program Manual (with particular attention to the specific functions of the Health Home Care Manager position). This manual was created in accordance with the COBRA and Health Home reimbursable procedures and based upon AIDS institute NYSDOH and Health Home guidelines 
  • Complete an in-depth screening, intake, and initial assessment; subsequently completing and overseeing an individual-focused care plan, detailed and mutually developed with each participant 
  • Implementation of the care plan with documented follow-up and monitoring of the plan 
  • Advocacy and knowledge of available and appropriate services for participants and their families/collaterals 
  • Conduct home visits as required and as needed (with a minimum of one home visit per care plan period) 
  • Conduct regular case conferences with outside providers (NOTE: Care Manager must ensure that at least one case conference is conducted at least once per care plan period for every client). 
  • Being available for crisis intervention, having to remain with a client until the issue is resolved or addressed 
  • Complete accurate and up to date documentation and consents for services. Conduct daily follow-up to address any issues, review the previous day's activities, and to discuss any patient issues and/or new insights regarding a particular client 
  • Be knowledgeable and up-to-date in areas pertaining to HIV / AIDS symptoms, treatment, medications, substance abuse, and other chronic conditions.
  • Be able to work in and create a positive environment, manage providers, delegate and oversee tasks/duties, and address conflicts as they arise.
  • Communicate effectively with supervisory personnel in a cooperative and supportive manner. 
  • Maintain monthly per member per month billing standards, meeting one of the 5 core Health Home functions. 
  • Complete HCBS Waiver Form if they do not meet the criteria to provide HCBS services
  • Complete all documentation related to HARP/HCBS services. Assessment for clients who are deemed eligible for HCBS services.  
  • Create an individual-centered plan of care that integrates behavioral and physical health services and includes HCBS for clients who are found eligible.
  • Coordinate with a client’s Managed Care Organization (MCO’s) in order to ensure a client receives the HCBS service(s) they are eligible for.

 

Qualifications/Criteria:

To perform the job of Care Manager successfully an individual must be able to perform each essential duty satisfactorily. The qualifications listed below are representative of the knowledge, skill and/or ability required.   • Minimum of one year of direct case management experience* 
  • Knowledge of HIV/AIDS, Substance use/abuse, mental, and other chronic illnesses (asthma, diabetes, high blood pressure, etc…) 
  • Meets physical and cognitive requirements needed to complete assigned tasks and duties of the position with reasonable accommodations being made for individuals with disabilities 
  • Ability to travel within the five boroughs of New York City via public transportation 
  • Ability to demonstrate proper judgment and decision making skills when necessary 
  • Excellent written, verbal, and organizational skills 
  • Ability to manage time effectively
  • Ability to input encounters with clients and/or providers on a daily basis
  • Bilingual in Spanish preferred 
  • Knowledge of computers/word processing a plus

 

Education: 

  • A minimum of a Bachelor's Degree from an accredited school in an appropriate human/social service and/or public health field 

 

* Direct Care Management experience includes verifiable full/part-time care management or case work with the following populations: those with HIV / AIDS, or a history of mental illness, homelessness, chemical dependence and/or other chronic illnesses. Experience with families or groups preferred; experience with HRA, HASA, hospitals or clinics is a plus. Qualifying experience does not include such activities or job positions such as pastoral counseling, counseling, therapeutic activities, internships or any unpaid/volunteer positions.


Salary is $50,000 annual


8:30am-4:30pm or 9am-5pm
