PrimaryOne Health is currently seeking a Behavioral Health Outpatient Clinician II for our Columbus health centers. 

Essential Duties and Responsibilities: 


Performs diagnostic and psychosocial needs assessments on adult, pediatric, obstetric, gynecologic, and geriatric patients.
Provides risk assessment and interventions on patients connected with behavioral health services.
Provides counseling services when deemed appropriate.
Works with the healthcare team to enhance patient-centered care received by the patient.
Provides education to the team regarding the patient’s behavioral health concerns of the effect of the patient’s social situation on their health.
Assists with linking patients to needed social resources, including advocating on their behalf with external agencies and institutions.
Works collaboratively with MSW internship training program.
May be responsible for attending various community meetings and/or events as a PrimaryOne Health representative as assigned.
Occasional travel to various PrimaryOne Health center sites required.
Other duties or special projects as assigned.


 

Successful candidates must possess a master’s degree in social work or counseling and a license to practice in the State of Ohio (LISW, LISW-S, LPCC, LPCC-S, LMFT, IMFT, IMFT-S). Two years’ experience in Behavioral Health (mental health and substance use); Integrated care, crisis intervention, or medical setting experience preferred (can include internship).
