Purpose: This is a full-time, 35-hour/week position. Under the direction of the Director, the Assistant Director will be second in command overseeing Community-based Care Management, Health Navigation, Resident Services and the Booker Center. The Assistant Director will work closely and collaborate with other internal departments to assist in identifying residents who need support and aid in reducing barriers related to the Social Determinants of Health.
Essential Job Functions:
- Manage the CBCM coordinators, Health Navigators, ROSS Coordinators and the Booker Center
- Communicate with a multi-disciplinary leadership team
- Coordinate outreach events
- Be available for consults
- Conduct monthly 1:1’s with staff
- Assist in stakeholder reporting
- Distribute referrals in a timely manner
- Provide guidance and coaching to new hires
- Implement programmatic changes based on need
- Regularly review shift reports/daily contacts
- Assist in documentation compliance
- Perform wellness checks as needed
- Coordinate and supervise interns from local universities
- Keeps the Director informed of program status, operational issues, staffing matters, and emerging risks through regular written and verbal reports, and prepares reports and data as requested
- Completed other duties as assigned by the Director and/or Executive Director
Knowledge/Skills/Abilities:
- Must report to work at the scheduled time, with few absences, and complete work promptly, accurately, and thoroughly.
- Community social services knowledge and experience are strongly preferred.
- In-depth knowledge of social service providers and programs available in Allegheny County.
- Ability to communicate professionally, both verbally and in writing, with customers and staff.
- Ability to interact with people from a broad range of social and economic backgrounds.
- Familiarity with dispute resolution concepts.
- Must possess and maintain a current, valid PA Drivers license and use of a personal vehicle, and be able to travel throughout Allegheny County.
- Proficient in the use of Adobe and Microsoft software (Windows; Outlook; Word; PowerPoint; Excel; Access).
- Hard working, dependable, well organized, energetic, highly motivated creative, and personable.
- Candidates must successfully pass a post-offer physical exam and drug screen, criminal background check.
Required Education/ Experience: Master’s degree in community or social services with 5 years experience. Licensure preferred (LSW, LCSW, LPC). Experience compiling and sorting data, entering raw data into database. Proficient in various programs to include, but not limited to: Microsoft Outlook, Word, and Excel. Demonstrated knowledge and experience in the provision of community social services and trauma-informed care. Must have and maintain a valid PA driver’s license and use of a personal vehicle. Must possess or obtain mandated reporter certification along with CPR and crisis intervention training. Must complete onboarding training modules prior to Go Live date.
Physical Requirements: Work is performed both in office settings and community settings. Must be physically able to access various physical locations on and off ACHA. Must have a level of manual dexterity sufficient to allow for the operation of a computer, telephone, fax machine, calculator, etc. Ability to move, handle or lift small objects around desk area, i.e. files, computer printouts, reports, calculator, office supplies, etc.
