This position offers a $2,000 (1 year stay-on agreement) or 4,000 (2 year stay-on agreement) Sign On Bonus with the agency!  This bonus will be provided upon completion of a probationary period.

Responsibilities: Exciting opportunity to join an integrated healthcare team serving people in our community, aged 5 and up in a primary care setting. As a Behavioral Health Consultant in this setting, you will be working with two primary care, multi-disciplinary treatment teams located within the Ingham Community Health Center, Forest location.  In this role, you will partner with healthcare providers, medical assistants, community health workers and others to improve the overall quality of life and health outcomes of patients served, providing mostly brief interventions aimed at functional restoration, short-term therapy focused on a clear, person-centered goal and comprehensive bio-psycho-social assessments.  Candidates for this position may have experience in SUD services, evidence-based trauma interventions, trauma informed care, motivational interviewing, solution focused therapy and other evidence-based practices.  Responsibilities include thorough and timely documentation, crisis assessment and intervention, understanding of Co-Occurring treatment, care coordination and collaboration with community partners, and other community programs, as well as understanding of referral processes and community resources for other supports. This position offers the opportunity to work with a diverse population who are traditionally underserved in primary care. Will act as a liaison between CMHA-CEI and Ingham Community Health Center (ICHC), a Federally Qualified Health Center. Professional development opportunities are offered. Responsible for carrying out all activities of the program in such a manner that fulfills the specific Health Care Setting, FQHC and CMHA-CEI’s mission, policies, and procedures.

 

Requirements: Possession of a Master's degree in Social Work is required as well as possession of a State of Michigan credential/licensure (LMSW, LLMSW or LPC specifically). If current limited license status, must be in the process and have full licensure within 2 years of hire. One year of professional experience specific to the provision of a brief model of outpatient therapy is required. Two years of independent clinical experience and the ability to practice independently, without on-site clinical supervision is required. Candidate will need to be credentialed with a variety of Medicaid Health Plans and Commercial Insurance. Ability to communicate accurately and effectively both in writing and verbally is required. Candidate must be able to work well with others. Candidate may be expected to work some evening hours. Access to reliable means of transportation for job related use is required.  

 

Conditional Employment Requirements: Employment offers for this position will be contingent upon the results of a background verification and credentialing procedure consisting of:  Michigan State Police clearance check, verification of a professional license, Medicaid/Medicare verification, educational background check, central registry clearance and verification of an acceptable work history.  Must be able to pass pre-employment drug screen.

Salary/Hours: $31.92 - $35.75 per hour with scheduled, annual increases that can result in an hourly rate of $41.50 per hour. 40 hours per week, some evening hours.
