Treasure Coast Community Health (TCCH), a long-established and highly respected healthcare provider and Federally Qualified Health Center with locations throughout Indian River County, FL is seeking a Wellness Coordinator to join our team.

 

The Wellness Support Coordinator facilitates, coordinates, and tracks the school health care programs referral network and processes. The Wellness Coordinator acts as the liaison between TCCH and Dodgertown Elementary relative to ensuring access to care for students at the school through a organized referral process. Additionally, the coordinator works to promote wellness within the staff and administration through a variety of initiatives. Finally, the coordinator works to ensure that all of the requirements for the Wellness Pillar (UCF Partnership School Accreditation) are a focus of the work done at the school.

 

Essential Duties and Responsibilities include the following. Other duties may be assigned.

  • Facilitate and coordinate all medical clinic referrals, including health, dental, vision and mental health referrals.
  • Track and follow up on all referrals made in and out of school.
  • Completion EPIC training and utilize for scheduling of patients, financial assistance and review of documentation when applicable.
  • Serve as the point of contact between TCCH and Dodgertown administration.
  • Serve as entry point to the Community Partnership School for students and
  • Coordinate Health & Wellness events for students, staff & community.
  • Participate in Intervention Team meetings to provide information on behavioral interventionsand relevant referrals as
  • Establish referral network/linkages with outside
  • Record professional assessments, treatment plans, and progressnotes.
  • Utilize the school's data management system for proper
  • Ensure that all families are evaluated for eligibility of TCCH financial programs if applicable, including completion of applications through the EPIC system/IRCHD portal. billing and other reimbursement
  • Participate in the quality improvement
  • Coordinate Health & Wellness events for students, staff &
  • Submit all documentation in a timely manner (data, trackers,).
  • Possible direct transport or arrange for transportation ofclients and accompanies the child.
  • Contribute to a positive, engaging work
  • Develop a strong knowledge base and stay current on job-related issues and
  • Participate actively in departmental meetings, training and education, as well as the quality
  • Comply with TCCH's code of conduct, policies, procedures and other
  • Pick up projects on the fly; perform other duties as assigned from time totime within the scope of responsibility of the position.
  • Submit all documentation in a timely manner (data, trackers, etc.)
  • Possible direct transport or arrange for transportation of clients.

 

Education/Experience:

  • Bachelor's degree in social work, psychology, public Health, counselling or Nursing from an accredited university or applicable experience.
  • Florida Driver's License within 30 days from hire is required, along with daily access to a reliable and insured vehicle.
  • One-year previous experience working with children and families, preferred.
  • Experience working in a school setting preferred.
