Support Coordinator

Care Lync is seeking a dedicated Support Coordinator to join our team. In this role, you'll serve as the primary point of contact for individuals on your caseload, ensuring they receive the services and support they need while keeping their voice and choices at the center of every decision.

What You'll Do

  • Get to know each person on your caseload and help build their personalized service plan (called a Plan of Services and Supports, or PSS)
  • Connect people with the right service providers and help them make informed choices
  • Check in regularly - by phone monthly, and face-to-face quarterly (plus at least one home visit each year)
  • Track Medicaid/waiver eligibility, submit renewals, and follow up on paperwork so nothing lapses
  • Keep accurate, timely notes and documentation in the state's LTSS system
  • Watch for and immediately report any signs of abuse, neglect, or exploitation
  • Educate the people you support and their families on their rights each year
  • Team up with other providers and care coordinators so nothing falls through the cracks
  • Travel to see the folks you support (up to 50% of your time)

What You'll Need

  • A bachelor's degree in a human services field (no experience required), or
  • A bachelor's degree in any field plus 1+ year of full-time experience working directly with people who have intellectual/developmental disabilities, other disabilities, or mental illness
  • Comfort with Microsoft Office and learning state databases
  • A valid driver's license, current vehicle insurance, and registration
  • Willingness to complete required training and certifications

A Few Things to Know

  • This role requires a national background check (including fingerprinting) before you can start working directly with people we serve
  • We verify degrees through official transcripts, not diplomas or letters
  • To avoid any conflict of interest, you can't be employed by, financially tied to, or serve on the board of another IDD service provider, and you can't be assigned to a family member or anyone you're already a paid caregiver for
  • You'll be considered a mandated reporter in this role
