  • Work with healthcare providers to ensure patients receive accurate and timely information
  • Maintain accurate patient records and manage interactions within CRM systems (e.g., Salesforce)

Drive engagement and follow-up

  • Maintain regular communication with patients and caregivers throughout their journey
  • Track patient progress and proactively follow up to ensure continuity of care
  • Support patients as they transition into advocacy and long-term engagement

Qualifications

  • Clinical background required (BSN, ADN, RN, Social Work, OT, PT, or DPT) with active licensure
  • Experience in a hospital, clinic, or patient care setting with a focus on patient education and care coordination
  • Strong communication skills with the ability to guide patients through complex healthcare decisions
  • Experience working with CRM systems (e.g., Salesforce) and standard business tools

Preferred qualifications:

  • Experience working with stroke, neurology, or rehabilitation patients
  • Background supporting patients with advanced or implantable medical devices
