As the Admissions Coordinator, this role will also carry out admissions functions including responding to program referrals, making updates to the interview and screening process, and interviewing candidates for the program.

Primary Responsibilities:


Oversee the review and assessment of information on all potential placements, the subsequent decision regarding acceptance, rejection or on-hold waiting list.
Responsible for acquiring funding information for each potential placement as well as current residents.
Oversee all aspects of the admissions process including contacting all referring sources (parent, clinician, caseworker, etc.) for updates on intake process, schedule on-site interviews and potential admission dates.
Provide tours of the Agency to potential clients, parents/guardians, and referring agents.
Maintain and manage all admission related information and documentation
Create all admission reports in a timely manner
Enter and manage admissions/client information in the ETO database
Enter/monitor/manage Homeless Management Information System (HMIS) data for the following programs:

RHYMIS-FYSB (Family and Youth Services Bureau) funded clients
CSF (Children’s Service Fund) funded clients
HUD (Housing and Urban Development) funded clients


Develop and maintain strong relationships with apartment communities and property management teams to foster ongoing partnerships and referrals.

Qualifications:


Bachelor’s Degree in Social Work, Counseling or related Health and Human Services
Experience with older youth and young adult population
Valid Driver’s License, dependable personal transportation, and valid insurance
At least 21 years of age


 

Job Knowledge, Skills and Abilities:  


Strong engagement skills with older youth and young adult population.
High level of organization.
Strong communication both verbal and written.
High level of personal accountability.
Ability to gather and analyze information to make quality assessments, and treatment plan recommendations.
Computer Skills: Proficient in Microsoft Office, ability to learn agency client. management database, and utilize virtual meeting programs such as Teams, Zoom etc. 
The physical requirements of this job include sitting, standing, walking, and climbing. stairs on a regular basis, lifting to 50lbs, pulling and/or pushing on occasion.

Working Environment:


Most of the work is performed in an office setting. May need to travel into the community occasionally.
