Position Description:


Transitional Living Program Case Manager: Walk Beside Young Female-Identifying Women as They Build a Life of Their Own


180 Degrees believes young people lead their own journeys, and the right partner can help them get where they're going. As a Transitional Living Program (TLP) Case Manager, you're that partner: a trusted, steady presence who helps young adults build stability, independence, and long-term housing.


You'll support one of our two TLP sites, Butterfly House, with back-up coverage for the other when needed:

  • Brittany's Place TLP: Congregate living with 5 bedrooms for female-identifying youth ages 16-19, licensed under MN Rule 2960, located on campus with Brittany's Place Shelter
  • Butterfly House TLP: Congregate living with 5 beds for female-identifying youth ages 18-22, located off-campus in St. Paul

Residents can stay up to 18 months, giving you time to build real relationships and see real progress. Both programs are funded in part by the FYSB Transitional Living Program.

This role centers authentic relationship, youth voice, and shared decision-making. You foster safety, belonging, and connection that support healing and forward movement. Youth lead. You walk alongside them, helping them explore options, understand systems, and build the skills to move toward self-sufficiency.


What you'll actually do:

  • Work 1:1 with residents to identify goals, strengthen skills, and move toward housing stability, employment, and financial wellness
  • Help residents navigate systems and build independent living skills
  • Use a trauma-responsive, culturally grounded, youth-driven approach in every interaction
  • Foster a sense of safety, belonging, and connection that supports healing and growth
  • Support youth voice and shared decision-making as residents lead their own journeys
  • Maintain accurate documentation in multiple databases, including HMIS, to meet FYSB funding requirements
  • Participate in the on-call rotation and work flexible hours, including evenings and weekends, based on residents' needs
  • Provide back-up coverage for the other TLP site when needed

This is a full-time position (1.0 FTE, 40 hours per week), onsite. No direct reports, but direct impact on every resident's path forward.


Qualifications: 

  • Minimum age: 21.
  • Bachelor’s degree in human services, psychology, social services, social work, or a related field and two years of related case management experience; case management experience may be substituted in lieu of education on a year-for-year basis.
  • Two year's experience in the nonprofit sector required.  
  • Valid driver’s license and insurance; dependable transportation for traveling to community partners and events; ability to drive Company vehicle without restrictions.
  • Ability to clear a Department of Human Services background screening.

Essential Competencies: 

  • Four years’ experience in the nonprofit sector preferred.
  • Three years of related case management experience in crisis, violence, trafficking, exploitation or homelessness preferred.
  • Strong foundation in trauma-responsive and youth-centered practice.
  • Ability to build rapport with youth and families while maintaining professional boundaries.
  • Skilled in care coordination, advocacy, and collaborative problem-solving.
  • Understanding of confidentiality, data practices, and ethical responsibilities.
  • Strong written and verbal communication skills and documentation skills.
  • Excellent organization, time management, and attention to detail.
  • Ability to operate effectively in fast-paced, complex environments.
  • Proficiency with computer systems and databases.

While performing the duties of this job, the employee is regularly required to use a computer. The employee is frequently required walk, stand, and go up and down stairs. Regular computer use. Combination of seated work and movement within the shelter and community. Occasional evening, weekend, or holiday hours, including on-call responsibilities. Travel to community partners and appointments as required.

Work Environment: 

This role moves with the rhythm of a program that never closes. 180 Degrees operates 24/7/365. Crisis doesn't run on a 9-to-5, and neither do we. Case Managers participate in the on-call rotation, and flexibility to work extended hours for events and special projects is part of showing up fully for this work. This is an on-site, awake position at our Brittany’s Place Transitional Living Program which serves female-identifying youth.
