Do you want to use your clinical skills to support individuals experiencing homelessness and help them maintain housing and achieve their goals? Join Health Care for the Homeless as a Therapist Case Manager and make a meaningful impact in your community.


As a Therapist Case Manager, you will provide direct mental health therapy and intensive case management services to adults experiencing homelessness with complex needs. You will partner with clients to support their health, housing stability, and personal goals through a trauma-informed, client-centered approach.


Key Responsibilities

  • Deliver behavioral health assessments, individual and group therapy, and intensive case management.

  • Conduct home and community visits to ensure client progress and well-being.

  • Collaborate with interdisciplinary teams and external partners to coordinate care.

  • Support clients with budgeting and financial literacy as part of representative payee services.

  • Maintain thorough and timely documentation in the electronic health record.


What We’re Looking For

  • Master’s degree in Social Work (LMSW or LCSW-C required).

  • At least 1 year of experience in therapy and case management for individuals with substance use and mental health disorders (preferred).

  • Knowledge of Harm Reduction, Motivational Interviewing, and Housing First practices.

  • Strong communication, organizational, and crisis intervention skills.

  • Valid Maryland driver’s license and access to a personal vehicle.
