What We’re Looking For


Trillium Health Resources has a career opening for a Transition to Community Living (TCL) Housing Supervisor to join our team! The TCL Housing Supervisor is responsible for overseeing a team of Housing Specialists who provide care coordination activities related to acquiring supportive housing such as treatment planning, assessment, monitoring, educating and referral/linking to members who are transitioning to supportive housing or who are at risk of eviction, or who are being evicted from their tenancies procured through TCL. The TCL Housing Supervisor ensures members receive whole person care that helps to establish positive outcomes. 


On a typical day, you might:


  • Provide direct supervision and oversight for assigned team.
  • Ensure the appropriate completion of Housing Assistance Payment (HAP) contracts to include monthly review of invoices. 
  • Monitor and reconcile housing data in the North Carolina Housing and Finance Agency (NCHFA) platform on an ongoing basis and ensures outstanding tenancy issues are addressed timely. 
  • Oversee the housing inspection referral process with 3rd party inspection vendor and leads meetings to provide updates on outstanding inspection referrals.
  • Provide technical assistance to Housing Specialists and landlords regarding specific member needs.

Qualifications


Required:

  • Bachelor’s degree in a Human Services field and five (5) years of experience working in care management, case management or care coordination with the MH/SU/BH population including experience working with individuals and the housing system serving this population to obtain and maintain permanent supportive housing; OR
  • Master’s degree in a Human Services field and three (3) years of experience working in care management, case management or care coordination with the MH/SU/BH population including experience working with individuals and the housing system serving this population to obtain and maintain permanent supportive housing.
  • Must have a valid driver’s license.
  • Must reside within Trillium’s catchment area of North Carolina, which includes the following counties: Anson, Bertie, Beaufort, Bladen, Brunswick, Camden, Chowan, Carteret, Columbus, Craven, Currituck, Dare, Duplin, Edgecombe, Gates, Greene, Guilford, Halifax, Hertford, Hoke, Hyde, Jones, Lee, Lenoir, Martin, Montgomery, Moore, Nash, New Hanover, Northampton, Onslow, Pamlico, Pasquotank, Pender, Perquimans, Pitt, Randolph, Richmond, Robeson, Sampson, Scotland, Tyrrell, Warren, Washington, Wayne, Wilson.
  • Must be able to travel within catchment as required.


Preferred:

  • Experience working with Transition to Community Living population and supporting individuals to apply for Targeting/Key, Section 8 and other housing subsidies.
