Are you passionate about helping others achieve their goals and live more independent, fulfilling lives? Join Starkey as a Targeted Case Manager and become an advocate, resource connector, and support partner for individuals with intellectual and developmental disabilities.

In this role, you'll work closely with individuals, families, guardians, and service providers to coordinate services, monitor outcomes, and ensure each person receives the support they need to thrive.

What You'll Do:

  • Coordinate and monitor services for individuals on your caseload
  • Advocate for individuals and help them access resources and supports
  • Build strong relationships with families, guardians, providers, and community partners
  • Assist with person-centered planning and goal achievement
  • Serve as the primary point of contact and referral source for assigned individuals
  • Ensure services are meeting needs and supporting independence, community integration, and quality of life

What We're Looking For:

Required:

  • Bachelor's degree in Human Services or a related field
  • OR five (5) years of related, verifiable full-time experience in developmental disability services in lieu of a degree
  • Minimum six (6) months of experience in developmental disability services
  • Valid driver's license with a good driving record
  • Ability to successfully complete the Targeted Case Management state test within the required timeframe

Preferred:

  • One (1) year of case management experience
