Supv Transitional Care | Case Management
UF HealthThe incumbent in this position plans, organizes and directs the Discharge Facilitators staff who are responsible for the operational activities of the Transitions of Care for Case Management department. Assures appropriate staffing and manages the financial performance of the area being managed.
Responsibilities
Responsible for:
- Completing job evaluations, hiring decisions, providing input for budgeting and meeting productivity goals. Responsible for developing post-acute relationships and fast contracting/invoicing.
- FAST program invoicing/tracking transportation. Accountable for strategic planning, implementation, evaluations, and revision of systems that support the Case Management Department and organizational goals. Maintains 24-hour accountability for his/her area functions.
Qualifications
Minimum Education and Experience Requirements: Bachelors Degree, LPN or equivalent education required.
- Three-five years care coordination/discharge facilitation (planning) experience applicable to the designated population served.
- Three years management/supervisory experience preferred. Transitional Care experience required.
- Minimum of three years care coordination experience required.
Must possess strong analytical, interpersonal, leadership and management skills with knowledge and application of professional practice environments, quality/safety improvement processes, health care delivery systems, and budgetary processes. Must be able to demonstrate skills in creating vision, effecting change and facilitating transitions.
Motor Vehicle Operator Designation: Employees in this position: Will operate vehicles for an assigned business purpose as a non-frequent driver NOTE: A frequent driver is defined as one who uses his/her personal or Shands automobile a) at least once daily, b) at least five individual trips per week or c) drives, on average, over 150 miles per week in the performance of his/her job.
