Position Summary:


Works as part of a multidisciplinary Coordinated Specialty Care team to help young individuals experiencing early psychosis achieve their academic and career goals. Provides personalized vocational assistance, case management, and skills training to help individuals secure mainstream education or competitive, integrated employment as outlined in the Texas Administrative Code and current state guidelines for individuals with a mental illness. This role heavily involves community outreach to build relationships with local employers and schools, and includes advocacy, crisis services, individual and group skills training, and other tasks as assigned by leadership in accordance with the philosophy, goals, and objectives of Texoma Community Center. Operates under trauma-informed and recovery-oriented models of care which align with CCBHC standards.


Qualifications:

Education and/or Experience:

  • Bachelor’s degree in Psychology, Social Work, Sociology, or Counseling required 
  • Licensed as a Qualified Mental Health Professional (Can be obtained during training period)
  • Master’s level intern with two (2) years Human Services experience preferred 
  • Experience working with individuals with mental illness and/ or addiction issues desirable

 

Certificates, Licensure, Registrations:

  • Current driver’s license and a vehicle for use at work



Knowledge, Skills and Abilities:

  • Ability to speak a second language relevant to population served preferred
  • Must be self-motivated, organized and detail oriented
  • Knowledge of Office Suite 
  • Consistently practices Trauma-informed Model of Care, understands and considers the pervasive nature of trauma.
  • The knowledge of cultural differences and ability to work with individuals from different cultural backgrounds.
  • Ability to speak a second language relevant to population served preferred

Essential Job Duties and Responsibilities




Provide case management and supported education and employment services


  1. Monitor clients in home and community setting
  2. Provide case management services
  3. Provide coordination and referral services
  4. Document services according to standards
  5. Provides support and skills related to education and employment 

Provide psychosocial rehabilitation 


  1. Teach basic living and social skills in natural setting
  2. Teach coping skills for managing symptoms and for daily living and interactions
  3. Document service delivery according
  4. Teach skills that will help person cope with emotional and behavioral problems
  5. Complete documentation that meets billing standards

Provide crisis-problem solving assistance as needed


  1. Evaluate problem
  2. Offer choices
  3. Assist to implement choice
  4. Consult with family
  5. Document service
  6. Provide follow up and aftercare

Maintain documentation


  1. Develop Care Plan
  2. Diagnostic Form routed for update annually or as needed
  3. Documentation must meet Medicaid standards for billing
  4. Fee Assessment Annually 
  5. Client Rights, HIPPA documents annually 
  6. Complete Assessments according to time standards

Compiles, understands, and enforces state and federal regulatory requirements related to the performance of TCC operations and requirements, including but not limited to HIPAA, the Code of Conduct, the Compliance Policy, Safety and Infection Control, all department and organization-wide policies, and Texas Administrative Code.

  1. Maintains confidentiality and follows HIPAA regulations.
  2. Maintains current and accurate documentation.
  3. Follows all TCC policies and procedures.
  4. Follows and knows safety policies and procedures.
  5. Attends all scheduled meetings and participates in these meetings.
  6. Considers the impact of regulations in work that is produced and discusses concerns appropriately to ensure compliance.

Participates in TCC committees, quality assurance, performance and quality improvement initiatives, and activities which support the facility and department operations.

Follows HIPAA regulations and ensures that the confidentiality of client medical, personal, financial records, employee records, and organizational records is maintained. 

Attends required training and maintains current status of required training; and performs other duties as assigned.
