Annual Salary: $50,604

Work Schedule: M-F 8:30a - 5:00p

Core Responsibilities

The Support Coordinator for Child & Adolescent Developmental Services ensures continuity of care for children and adolescents with intellectual and developmental disabilities by coordinating services and supporting informed, person-centered decisions.

Key responsibilities include developing individualized, person-centered plans; connecting clients and families to appropriate providers and community resources; monitoring service delivery and progress toward outcomes; advocating for needs across systems; and supporting community integration across medical, educational, vocational, and recreational domains in alignment with discharge protocols.

Required qualifications:
- Bachelor’s degree in Human Services, such as Social Work, Psychology, or Counseling 
- One (1) year of experience in support coordination services with the assigned population 

A Typical Day

Your day is primarily onsite working within a collaborative, professional team environment focused on service excellence and empathetic care. You’ll balance scheduled touchpoints with families and providers with time to update records, review plan milestones, and prepare for upcoming meetings. Throughout the day, you’ll navigate a fast-moving service landscape—shifting between problem-solving, coordinating next steps, and ensuring families have clear, practical information to make informed choices. You’ll also spend focused time tracking progress toward outcomes and confirming that supports remain aligned as needs change, helping maintain steady momentum toward successful transitions and discharge goals.
