Work schedule: Monday - Friday 8:30 am - 5:00 pm
Make a meaningful difference in the lives of adults with intellectual and developmental disabilities (IDD) as a Support Coordinator with the Hampton-Newport News Community Services Board (H-NNCSB). In this role, you will help individuals access the services, resources, and supports they need to live as independently and successfully as possible in the community.
Minimum Requirements
- A Bachelor's degree in Human Services, such as Social Work, Psychology, Counseling, or a related field.
- One year of experience providing case management or support coordination services to adults with developmental disabilities.
What You'll Do
- Develop and monitor individualized service plans based on each person's needs, strengths, preferences, and goals.
- Coordinate medical, behavioral health, residential, employment, transportation, and community-based services.
- Advocate for individuals and provide ongoing support, crisis intervention, and assistance with daily living needs.
- Collaborate with individuals, families, healthcare professionals, and service providers to ensure appropriate and effective supports.
- Promote independence, self-determination, community inclusion, and quality of life while maintaining accurate documentation.
