Our Street Health team bridges the gap between vulnerable community members and the care and resources they need. Through field-based outreach, engagement, and Enhanced Care Management (ECM), we help individuals access healthcare, behavioral health services, housing resources, and supportive services that promote improved health outcomes and greater stability. Every day, this team builds meaningful relationships, removes barriers to care, and helps individuals connect to services that support long-term wellbeing.
What Success Looks Like
This is a community-based leadership role that combines staff supervision, program oversight, care coordination, case management, healthcare navigation, and operational accountability. Successful ECM Supervisors are approachable, organized, and committed to coaching staff while building trust with individuals who may be disconnected from traditional healthcare and social service systems. They are comfortable guiding teams in the field, supporting complex member needs, and collaborating closely with healthcare providers, case managers, managed care plans, and community partners.
Success in this role means leading a high-performing ECM team that delivers effective, person-centered care coordination while maintaining strong quality, compliance, and documentation standards. You’ll help strengthen care across multiple systems, support staff growth, remove barriers to service delivery, and ensure members are connected to critical resources that support their health and housing goals. Through strong leadership, collaboration, and follow-through, you’ll help improve access to care, continuity of services, and positive outcomes for individuals with complex medical, behavioral health, and social service needs.

Work Schedule
This is a full-time position that combines administrative leadership, field-based program oversight, staff supervision, and limited direct member care activities. The role is based within the Street Health Program and includes work in community, clinic, office, and outreach settings. While the typical schedule is Monday through Friday, operational needs may require occasional schedule flexibility to support staff, members, community partnerships, and program priorities.
Compensation & Benefits
The anticipated starting pay range for this role is $70,304 to $81,412 annually, paid out bi-weekly. Final placement and starting salary will be determined based on the candidate's experience, qualifications, certification status, and internal equity considerations.
