Role Overview

As the Supervisor of Integrated Case Management for Population Health, you will oversee daily case management operations, guiding a team of professionals to deliver collaborative, patient-centered care. Your role is critical in ensuring seamless assessment, planning, and coordination of services to meet the comprehensive health needs of individuals and families, promoting safety, quality, and cost-effective outcomes.

Qualifications

What You Will Need

Required Qualifications

  • Bachelors degree in nursing or related field (e.g., social work, counseling, health education) or Masters Degree in Social Work.
  • Three (3) years of clinical experience in case management, disease management, health coaching, nursing, or home health care.
  • Strong communication, interpersonal, analytical, and problem-solving skills.
  • Registered Nurse (RN) or Licensed Social Worker (LMSW) with a valid, unrestricted State of Michigan license.

Preferred Qualifications

  • Certification in Case Management (CCM) from the Commission for Case Management Certification (CCMC).
