Summary

Under general direction of case management leadership, the Case Management Supervisor position is responsible for serving as a clinical resource and unit expert for staff. This position links nursing leadership and the hospital medicine team with discharge planning processes and resources. The Supervisor will have responsibility for planning, directing, coordinating and evaluating case management roles as they directly interact with other hospital departments and community resources. This position involves interaction with physicians, nursing staff, case management, and community resources. The Case Management Supervisor is responsible for identifying process concerns, providing service recovery, and escalates issues to appropriate chain of command as needed. Active participation in team meetings, staff schedules, and education of the staff is a key role.


Responsibilities

(40%)* Assesses competency of department staff, and functions as a liaison to facility staff completing and maintaining department competency. Act as a resource and role model for unit practice, develops and maintains collaborative relationships and channels communication with members of multi-disciplinary care teams, including essential service leadership. Helps in development and delivery of process improvement strategies, and assist in promotion of teamwork and safe, timely discharge planning. Focus on developing department staff competency, technical skills, clinical decision making and professional practice for staff.

(20%)* Collaborate with medical staff department liaisons to develop quality improvement approaches and strategies to enhance delivery and utilization of health services and to share information relating to quality of care.

(30%)* In addition to supporting daily operations, the Case Management Supervisor will promote staff education through in person, one on one education, and will plan routine staff meetings to maintain department proficiency. Demonstrates ability to delegate responsibilities, make sound judgements and accurately assess when to refer issues to manager. 

OTHER DUTIES AND RESPONSIBILITIES:

Completes all training expected of staff, continues professional development and demonstrates expertise. Uses critical thinking in all interactions. Ability to identify problems, collects data, and establishes facts in order to effectively resolve problems. Identifies needs, assists in development and implementation of programs in corporation with Department of Education for orientation, staff development, and continuing education. Maintains process improvement programs for Case Management. Assures compliance with all policies, regulations, and advanced practices pertaining to Case Management and discharge planning. 

Completes schedules for department staff to ensure adequate coverage of department. Builds professional relationships with multidisciplinary teams, external facilities, and community resources. Demonstrate exceptional Customer Service skills. Completes Job Performance Reviews (JPRs) for direct reports. Sets realistic short and long range performance goals and evaluates progress. All other duties as assigned.

Required Education

Education: Bachelors Degree

Comments: Social Work or Nursing degree preferred.

EXPERIENCE, TRAINING AND SKILLS:

Case management experience obtained through direct work experience.

Knowledge of case management practices and procedures as normally required through 1-3 years In a Medical Center or similar setting.

Knowledge of Pathophysiology and Disease Process Basic computer and courteous phone skills.

Demonstrated conflict resolution, interpersonal, critical thinking and problem solving skills required.

Knowledge of Commercial and Government Payer guidelines. 

PHYSICAL/MENTAL REQUIREMENTS AND TYPICAL WORKING CONDITIONS

Exposure to stressful situations, including those involving public contact, as well as, trauma, grief and death.

Able to wear personal protective equipment that includes latex materials or appropriate substitute if required for your position.

Is able to move freely about facility with or without an assisted device and must be able to perform the functions of the job as outlined in the job description.

Overall vision and hearing is necessary with or without assisted device(s).

Frequently required to sit/stand/walk for long periods of time. May require frequent postural changes such as stooping, kneeling or crouching.

Some exposure to blood borne pathogens and other potentially infectious material. Must follow MyMichigan Health bloodborne pathogen and TB testing as required.

Ability to handle multiple tasks, get along with others, work independently, regular and predictable attendance and ability to stay awake.

Overall dexterity is required including handling, reaching, grasping, fingering and feeling. May require repetition of these movements on a regular to frequent basis.

Physical Demand Level: Light. Must be able to occasionally (33% of the workday) lift or carry 20 lbs., frequently (66% of the workday) 10 lbs. and or Walk/Stand/Push/Pull of Arm/Leg controls.
