SUD Case Manager

Job DetailsJob Location: Delaware, OH 43015Southeast is seeking a mission driven individual to help patients achieve long-term recovery and improve their overall well-being. We are seeking a skilled Substance Use Disorder Case Manager to join our team and make a meaningful impact on the lives of those we serve. Your role will include assessing patient needs, developing personalized treatment plans, coordinating services, and advocating for patients to ensure they receive comprehensive support throughout their recovery journey.

Key Responsibilities:


Conduct initial assessments to determine patient needs and create individualized treatment plans.
Provide case management services to patients, ensuring they receive appropriate care and resources.
Advocate for patients and collaborate with treatment teams, healthcare providers, and community agencies.
Monitor patient progress and provide ongoing support.
Help patients access resources such as housing, employment, and social services.
Educate patients about substance use disorders and available treatment options.
Document and maintain accurate progress notes.
Facilitate group or individual counseling sessions, if required.


 
Qualifications
Bachelor’s degree in social work, Psychology, Counseling, or a related field required.
Certification or licensure in substance use counseling required.
Prior experience working with individuals affected by substance use disorders.
Strong knowledge of addiction treatment modalities and community resources.
Excellent communication, interpersonal, and organizational skills.
Ability to work independently and as part of a multidisciplinary team.
Valid driver's license and insurance.
