Apply for a Clinical Rotation at Primary Health Care

Primary Health Care is a Federally Qualified Health Center, which means our patients come to us from every direction and every circumstance. Students who rotate here see real community health: complex cases, team-based care, and the kind of visits that stay with you. 

We accept Physician Assistant, Nurse Practitioner, Dentist, and Social Work students.

Before you apply, a few things to know:

• Your school needs an active affiliation agreement with PHC. If you're not sure whether one exists, ask your faculty advisor/program director before you apply.

• You will need to complete our onboarding & compliance requirements before your first day.

• We ask that you apply at least 60 days before your desired rotation start date. 

• Placement is subject to preceptor availability, which is limited, so we can't guarantee a spot but will review your needs and support them if we are able!

What happens after you apply: We'll confirm your school's agreement and your requested dates, and work to match you with a preceptor. Once that's set, we'll set a tentative start date and walk you through the requirements for onboarding!
