Purpose of the Role:
The SPOA Care Coordinator works under the Housing Department umbrella to manage Single Point of Access cases, facilitating housing placements for individuals with mental health and housing needs. This role ensures that clients are connected with appropriate housing resources through the SPOA system.
Key Responsibilities:
· Coordinate the intake, assessment, and placement of clients through the SPOA system.
· Work with the Housing Department and external agencies to match clients with suitable housing options.
· Facilitate communication between clients, housing providers, and community partners.
· Maintain accurate and up-to-date case records in compliance with SPOA guidelines.
· Advocate for clients throughout the housing application and placement process.
· Provide follow-up support to ensure clients remain stably housed.
· Special projects and other duties as assigned.
Qualifications for this Role:
· Bachelor’s degree in social work, psychology, or a related field required.
· Experience working with mental health and housing services.
· Familiarity with the SPOA system and housing resources.
· Strong organizational and case management skills.
