JOB DESCRIPTION:

 

The Socially Necessary Case Manager will work with the biological family to help them reach their goals in order to reunify with their children. In order to reach the family goals given by DHS and the court, a case plan will be followed. This role provides support and resources needed to achieve permanency. Case Managers will work with clients in their home or the community (doing services in the office is with special permission only).

 

PRINCIPAL DUTIES AND RESPONSIBILITIES:

 

INTAKE

•        Ensure appropriate referrals are received from DHS and work with administrative personnel to get units before services begin

•        Ensure appropriate intake documentation is received from DHS and completed with the family before services delivery

•        Ensure initial legal documentation is obtained prior to placement or service delivery

•        Refer client to applicable support services, i.e. behavioral health, medical, employment, etc.

 

ONGOING TREATMENT/DISCHARGE

•        Complete the intake assessment with the family in order to create a plan for services in the first week of case opening. Utilize interviews with the family, the state/county worker, and case documents for the assessment.

•        Complete the Master Service Plan with the family within the first week of the case opening. Ensure use of state/county/court requirements are a part of the plan.

•        Review the service plan every 90 days with the family to ensure goals are being met.

•        Carry out services in line with the family’s service plan.

•        Provide transportation as needed

•        Participate in service meetings which may include community support representatives, consumer’s guardian, biological family, state/county worker, outpatient clinician, foster parents, school system, and other invested parties

•        Maintain confidentiality as outlined in policy

•        Provide input and/or assist in MDT

•        Report to supervisor or to designated authorities any instances of non-compliance with OVP, regulatory rules/policies, reports of abuse or suspected abuse, neglect, or suicidal ideations – immediately

•        Upon discharge from the company, complete discharge paperwork within 48 hours.

 

CRISIS PREVENTION/MANAGEMENT

•        Participate in weekly rotating “on-call” schedule to provide on-call advisory and support to the families.

•        Document and report any on-call activities and follow-up with leadership

•        Report to supervisor or to designated authorities any instances of non-compliance with OVP, regulatory rules/policies, reports of abuse or suspected abuse, neglect, or suicidal ideations - immediately

•        Ensure all critical incidents are staffed with leadership and entered into the Electronic Health Record within established timeframes.

•        Ensure complete and accurate data is entered into the Electronic Health Record in accordance with state rules and regulations and company protocol, i.e. services, critical incidents, etc.

•        Ability to read, analyze, and interpret human service periodicals, professional journals, technical procedures and governmental regulations.

•        Ability to effectively present information and respond to questions from groups of managers, clients, customers and the general public.

•        Ability to define problems, collect data, establish facts and draw valid conclusions.

•        Organizational, and Written/Oral Communications skills

•        Must have the skills necessary to navigate and execute several software programs (i.e. Microsoft offices, Databases, Electronic Health Records, etc.)

•        Ability to adhere to scheduled and unplanned deadlines

 

ESSENTIAL DUTIES AND RESPONSIBILITIES:

 

•        Transfer established/stable caseload and foster homes to new Case Managers and mentor them through the transition

•        Ensure that foster parents and case managers needs are met during staff periods of transition

•        Perform other duties and responsibilities as assigned by immediate supervisor

•        Participate in the performance quality improvement process and execute any assigned tasks related to the process

•        Ensure that all employee-related documents are current and up to date for your employee personnel file

•        Complete and review documentation in the Electronic Health Record to ensure that it meets all state, licensure, accreditation, and company requirements within company timelines.

•        Ensure documentation is submitted to the appropriate agencies monthly per contract.

•        Ensure tracking of services provided and units and submit a report monthly for billing.

•    This position is community based.

•    Car insurance required.
•    Mileage reimbursed.


POSITION TYPE/EXPECTED HOURS OF WORK: 

This is a full-time salary position, and core hours and workdays are Monday through Friday with some evening hours. Create your own schedule based on required time in the office and client/foster parent availability. This position may require weekend work. 

There is an on-call aspect to this role that will be one week at a time, rotated each week by team members. This is answering after hours calls and could require going to the home or community for crisis management. 


WORK ENVIRONMENT: 
This job operates in an OVP Health location periodically, but most of the work will be in client homes and the community.
