Detailed Overview
Reporting administratively to the Manager, Clinical Operations and clinically to the Practice Leader at the site, the Social Worker South Asian Shapedown Program provides comprehensive therapeutic counselling to child/adolescent/families; conducts comprehensive psycho-emotional and social assessments of child/adolescent/families using a variety of modalities and in accordance with Social Work Professional standards, clinical policies, and practices, educates and provides consultations to child/adolescent /families and the healthcare teams; provides individual and/or group therapy/counselling to Program participants; participates in research, quality improvement and risk management projects and the development of community models, protocols and psycho-educational materials for the treatment of obesity disorders benefiting from Social Work intervention.
Responsibilities
- Plans and organizes the provision of Social Work to the South Asian Shapedown Program and participates in consultation with the South Asian Shapedown Program Team and the Social Work Practice Leader; participates in team activities as required for the Program's delivery.
- Conducts comprehensive psycho-emotional and social assessments of child/ adolescent/families particularly regarding child/adolescent screening for mood disorders, disordered eating behaviours and self esteem issues, by methods such as interviewing the child/adolescent/family, obtaining relevant information, gathering social data, regarding the child/adolescent and/or family and interpreting the data which includes reviewing paper and pencil rating scales, on-line questionnaires that are completed by the child/adolescent and family separately which supplements information attained in interviews.
- Assesses the readiness of child/adolescent/families to make lifestyle changes and to participate in care planning.
- Consults and communicates with community based health professionals, hospital staff or staff from other health agencies to enhance and provide continuity of care for South Asian Shapedown Program patients/families.
- Educates and provides consultation to child/adolescent /families and healthcare professionals on psycho-emotional and social issues related to the treatment of obesity disorders.
- Maintains child/adolescent's records by methods such as documenting patient assessments, charting patient information, preparing progress notes and reports in accordance with Social Work regulatory and Fraser Health established standards, policies and procedures.
- Communicates with community health and social agencies including government Ministries, by performing duties such as collaborating in the assessment and management of individuals at risk linking or referring to applicable resources when longer term treatment or follow up is required.
- Acts as a resource to child/adolescent and their families by performing duties such as providing information pertaining to navigating community resources and accessing these resources, and facilitating the development of community resources to support the health of families.
- Maintains a variety of records such as workload statistics by methods such as recording information into approved formats, collating data, analyzing trends, accounting for variances, preparing reports as required and recommending an action plan to the South Asian Shapedown Program Team and the Social Work Practice Leader.
- Assists the South Asian Shapedown Program Team by providing input to program planning and review of treatment protocols and/or interventions for the South Asian Shapedown program.
- Communicates psycho-emotional and social care and South Asian Shapedown program principles to members of the health care team, colleagues, child/adolescent /families through in-service education or orientation/ training sessions.
- Participates and collaborates on approved program research projects such as surveys and outcome evaluation by providing input into the planning and formulation of the research question, collecting data, integration and interpretation of data and recommending and identifying implications of finding for program delivery.
- Participates in program based quality improvement and risk management initiatives by methods such as collecting data and monitoring variances to expected outcomes. Makes recommendations based on documented results and implements approved changes.
- Performs other related duties as assigned.
Education and Experience
Master's Degree in Social Work from an accredited School of Social Work.
Three (3) years' recent related clinical experience with individual family, and group Social Work.
Current full registration with the BC College of Social Workers.
Current valid BC Driver's License and access to personal vehicle for business related purposes.
Skills and Abilities
- Comprehensive knowledge and understanding of psycho-emotional and social issues related to child/adolescent and families with disordered eating behaviours, other related mental health issues and family system issues.
- Demonstrated knowledge and understanding of therapy/counselling skills, therapeutic modalities and clinical practice models as it relates to individuals, families and groups.
- Ability to provide leadership to the Interprofessional team in care planning with complex situations.
- Ability to work effectively with others including working independently, or as part of Interprofessional team and understand the framework of other professions.
- Linking to resources applicable to needs of child/adolescent and their families; working collaboratively with other community agencies.
- Demonstrated ability to communicate effectively both verbally and in writing with child/adolescent and their families and other members of the Health care team and community agencies.
- Ability to communicate effectively in English and Punjabi verbally.
- Demonstrated ability to effectively manage conflict.
- Ability to facilitate groups, including group meetings, processes and decision making.
Applicants with a Bachelor of Social Work (BSW) degree from an accredited School of Social Work, and a demonstrated equivalent combination of education, training and experience may be considered.
