Make a difference in the lives of others by working as a Social Worker at CentraCare’s Sauk Centre Hospital! We are seeking a compassionate, collaborative Social Worker to join our interdisciplinary care team. In this role, you will assess the needs of patients and families and help connect them with the services, education and resources they need to successfully navigate their care.
Using independent judgment and close collaboration with providers, care coordinators, and other members of the healthcare team, you will advocate for patients, promote their safety and well-being and proactively identify opportunities to address barriers to care.
Responsibilities include:
- Meet with patients and families to identify strengths, assess needs and determine appropriate resources and support.
- Provide education and emotional support related to grief and loss, lifestyle changes, chronic illness and other life transitions.
- Connect patients and families with appropriate healthcare and community resources and serve as an advocate throughout the process.
- Conduct scheduled patient and family appointments based on individual needs.
- Collaborate closely with primary care providers, care coordinators, clinic staff and other members of the care team.
Schedule
- Part-time | 32 hours every two weeks | Sauk Centre Hospital
- Two days per week | Various shifts Monday through Friday | Typically working 8:00 am - 4:30 pm
Pay & Benefits
- Pay based on experience and degree
- Bachelor’s in Social Work starting at $30.65 per hour
- Master’s in Social Work starting at $33.81 per hour
- Pay range: $30.65 - $50.72 per hour

Qualifications

Bachelor's degree in Social Work and currently a Licensed Social Worker (LSW) in Minnesota
OR
- Master's degree in Social Work and currently a Licensed Graduate Social Worker (LGSW) in Minnesota
- Experience with the diversity of the local community is preferred
