We are seeking a highly skilled and motivated Master's Level Social Worker to provide clinical social work services in a fast-paced healthcare environment. This casual position primarily covers evening, overnight, weekend, and holiday shifts, requiring the ability to work independently while managing complex psychosocial situations and competing priorities.
The ideal candidate is an experienced social worker with exceptional assessment skills, strong clinical judgment, and the ability to make critical decisions with minimal supervision. This role requires flexibility, adaptability, and confidence in addressing urgent patient and family needs across a variety of clinical settings.
Provide services in accordance with the policies, procedures and guidelines established within the department, organization, and regulatory bodies.
Responsibilities:
- Conduct comprehensive psychosocial assessments for patients and families presenting with complex medical, behavioral health, social, and environmental concerns.
- Utilize advanced clinical judgment and critical thinking skills to identify risks, barriers to care, and appropriate interventions.
- Assess for safety concerns, including abuse, neglect, self-harm risk, domestic violence, substance use, and social determinants of health.
- Independently manage high-volume caseloads while responding to emergent and time-sensitive patient needs.
- Provide crisis intervention, emotional support, counseling, and resource coordination.
- Collaborate effectively with interdisciplinary team members to develop and implement patient-centered care plans.
- Facilitate referrals to community resources, behavioral health services, long-term care providers, and other supportive programs.
- Maintain accurate, timely, and comprehensive documentation in the electronic medical record.
- Demonstrate sound decision-making and problem-solving in situations with limited supervision and rapidly changing circumstances.
Requirements:
- Master’s degree in social work from a program accredited by the Council on Social Work Education (CSWE)
- LMSW/LCSW licensure in the state of Delaware required.
- 6 months social work experience
