Social Worker - LMSW PRN

We are seeking an experienced Hospice Social Worker (MSW) to join our interdisciplinary team and provide compassionate psychosocial support to patients and families navigating end-of-life care. This role delivers coordinated care in the home setting and serves as an advocate, counselor, educator, and resource for patients and caregivers.

As a Hospice Social Worker, you will:

• Complete and document the Comprehensive Assessment within 5 days of hospice admission to evaluate psychosocial, emotional, financial, environmental, and community resource needs.
• Develop and implement interventions that support patient goals and align with the individualized Plan of Care (POC).
• Provide counseling and emotional support related to end-of-life care, grief, loss, family dynamics, and adjustment.
• Assist patients and families with advance directives, funeral planning, discharge planning, and care transitions.
• Coordinate community resources and connect families with available support services.
• Participate as an active member of the Interdisciplinary Group (IDG) through collaboration and care planning.
• Maintain timely and accurate clinical documentation.
• Promote dignity, respect, patient rights, and compassionate service throughout the care experience.

Qualifications:

✔ Master’s Degree in Social Work (MSW) from an accredited program
✔ Current Social Worker License 
✔ Minimum 1 year of Hospice Social Work experience required
✔ Strong understanding of hospice philosophy and end-of-life care
✔ Valid Driver’s License and current Auto Insurance
✔ Current AHA CPR Certification
